摘  要:目的 探讨社区2型糖尿病患者(T2 DM)并发微血管病变的危险因素.方法 于2014年9月至2015年4月对上海市6个社区的2型糖尿病患者进行调查,收集5078例患者的人口学资料、临床资料和并发症检查结果,采用分类树模型对危险因素进行分析.结果 单纯T2 DM 2397例,合并糖尿病性视网膜病变(diabetic retinopathy,DR)1007例,患病率为21.1%(1007/4764),合并肾脏病变(diabetic nephropathy,DN)1937例,患病率38.4%(1937/5050).构建T2DM合并DR和DN的分类树模型,T2DM合并DR的模型筛选出糖化血红蛋白(HbA1c)、空腹血糖(FBG)、餐后2 h血糖(PBG)、病程、起病年龄、TG共6个危险因素变量,其中与结局关系最密切的是HbA1c、PBG和病程;T2DM合并DN的模型筛选出HbA1c、FBG、合并高血压、PBG、体质指数(BMI)、TG、高密度脂蛋白(HDL)共7个危险因素变量,其中与结局关系最密切的是HbA1c、FBG和合并高血压.结论 HbA1c是微血管病变的最主要危险因素,且截点值高于目前的控制标准,FBG、PBG是微血管并发症的独立危险因素,但对于不同类型微血管并发症的影响不同,病程和合并高血压分别是DR和DN的危险因素.Objective To analyze the risk factors of microvascular complications in patients with type 2 diabetes mellitus (T2DM).Methods The demographic and clinical data of 5078 T2DM patients in six communities of Shanghai were collected during September 2014 to April 2015. The risk factors of microvascular complications in T 2DM were analyzed by classification tree model .Results Among 5078 T2DM patients there were 1007 cases of diabetic retinopathy (DR) (21.1%) and 1937 cases of diabetic nephropathy (DN) (38.4%).The classification tree models showed that the risk factors of DR were higher hemoglobin A1C ( HbA1c), fasting blood glucose ( FBG), postprandial blood glucose ( PBG) and triacylglycerol ( TG) levels;longer course of disease and younger age of onset .The model showed that the risk factors of DN were higher HbA1c, FBG, hypertension, PBG, body mass index ( BMI ) and triacylglycerol (TG) levels;and lower high-density lipoprotein (HDL) level.The HbA1c, course of disease and PBG were more closely related to DR and HbA 1c, hypertension and FBG were more closely related to DN.Conclusion HbA1c is the most important risk factor to microvascular complications; FBG and PBG are independent risk factors of microvascular complications;the course of disease and hypertension are risk factors of DR and DN , respectively .
