臀部巨大表皮囊肿  

Giant hip epidermoid cyst:a case report

摘　　要：报告1例臀部巨大表皮囊肿。患者男,61岁。右侧臀部肿块逐渐增大30年余。皮肤科检查:右臀部可及一肿块,半球形,约12 cm×10 cm,表面色素沉着,质地较软,皮温正常,无溃疡及渗出,压痛(-)。皮损组织病理检查:臀部表皮囊肿,可见囊壁为表皮样上皮,伴有颗粒层及吞噬角化物的多核巨细胞反应,周围间质内散在慢性炎症细胞浸润及含铁血黄素沉积。诊断:臀部表皮囊肿。患者在连续硬膜外麻醉下行右侧臀部肿块切除术,术后恢复良好。A case of a 61-year-old male with giant hip epidermoid cyst was reported. The patient was admitted to our de- partment for appearance of a growing lump found on the right hip lasted more than 30 years. Physical examination revealed a hemispherical lump about 12 cmxl0 cm found on the right hip with skin pigmentation, soft texture, normal skin tempera- ture, no ulcer and no tenderness. After the removal of the surgical eontraindication, the patient underwent a lumpectomy of the right hip under the continuous epidural anaesthesia and his recovery was well. Histological examination of the lump re- vealed hip epidermoid cyst with cystic wall of epithelioid epithelium, accompanied by granular layer and phagoeytosis of the muhinucleated giant cell reaction. The chronic inflammatory cell infiltration and hemosiderin deposition can be seen in the surrounding cell interstitial. The diagnosis of hip epidermoid cyst was made.
