血清降钙素原联合C-反应蛋白对肝癌患者介入术后感染的临床诊断价值研究

摘　　要：目的探讨血清降钙素原(PCT)联合C-反应蛋白(CRP)对肝癌患者介入手术治疗后感染的诊断价值。方法选取2014年7月-2016年7月医院行介入手术治疗的肝癌患者88例为研究对象,根据术后是否感染分为两组,发生感染的28例患者纳入研究组,未发生感染的60例患者纳入对照组,抽取两组静脉血检测血清PCT、CRP水平、阳性表达情况及阳性、阴性预测值并进行比较。结果研究组血清PCT、CRP水平分别为(55.47±12.58)ng/ml、(2.18±0.42)ng/ml,均明显高于对照组(17.56±5.23)ng/ml、(0.76±0.45)ng/ml,差异有统计学意义(P<0.05);研究组血清PCT、CRP阳性率高于对照组,差异有统计学意义(P<0.05),血清PCT阳性、阴性预测值高于对照组,差异有统计学意义(P<0.05)。结论血清PCT、CRP对肝癌患者介入术后感染具有一定诊断价值,两者联合检测效果更佳,值得临床积极应用和推广。OBJECTIVE To explore the clinical value of serum procalcitonin（PCT）combined with C-reactive protein（CRP）in diagnosis of infections in liver cancer patients after interventional surgery.METHODS A total of 88 patients with liver cancer who underwent the interventional surgery in the hospital from Jul 2014 to Jul 2016 were recruited as the study subjects and were divided into two groups according to the status of postoperative infections,28 patients who had the infection were assigned as the study group,and 60 patients who did not have the infections were set as the control group.The venous blood specimens were drawn from the two groups of patients so as to detect the levels of serum PCT and CRP,the positive expression,positive predictive value and negative predictive value were compared between the two groups of patients.RESULTS The serum PCT level of the study group was（55.47±12.58）ng/ml,significantly higher than（17.56±5.23）ng/ml of the control group;the serum CRP level of the study group was（2.18±0.42）ng/ml,significantly higher than（0.76±0.45）ng/ml of the control group（P〈0.05）.The positive rates of serum PCT and CRP of the study group were higher than those of the control group（P〈0.05）;the positive,negative predictive value of the serum PCT of the study group was significantly higher than that of the control group（P〈0.05）.CONCLUSION The serum PCT,CRP has certain value in diagnosis of postoperative infection in liver cancer patients undergoing the interventional surgery,the combined detection of the serum PCT and CRP can achieve better effect,and it is worthy to be promoted in the hospital.
