摘 要:目的探讨曲面重建与最小密度投影技术在胰胆管梗阻性疾病中的联合应用价值。方法40例临床拟诊胰胆管梗阻性疾病的患者行cT检查,获得胰胆管及病变MPR、MinIP及cPR-MinIP三组图像,由2位高年资放射科医师对图像按“优、良、差”作3级评价。结果2位阅片者的评价意见具有良好的一致性(Kappa值为0.786)。MPR组与MinIP组间差异无统计学意义(P〉0.05)。CPR-MinIP组与MPR、MinIP组间差异有统计学意义(P〈0.05)。结论采用CPR-MinIP技术并选择合理的投影块厚度,能够为胰胆管病变的诊断和鉴别提供更清晰、有价值的影像学信息,该技术为胰胆管疾病的影像检查提供了一种方便、可靠、快捷的二维图像后处理手段。Objective To evaluate the application value of CPR-MinIP in obstructive diseases of pancreatic and bile duct. Methods CT examination was performed in 40 patients with clinically suspected obstructive pancreatic and bile duct disease. The MPR, MinIP and CPR-MinIP images of the pancreatic and bile duct lesions were obtained in three groups. The images were evaluated by 2 senior radiologists for 3 grades: excellent, good and poor. Results The evaluation opinions of the 2 readers were in good agreement ( k=0.786 ) . There was no statistical difference between the MPR group and the MinIP group (P〉0.05 ) . The difference between the CPR-MinIP group and the MPR group and the MinIP group was statistically significant (P0〈0.05 ) . Conclusions Using CPR-MinIP and selecting reasonable thickness of projection block can provide clearer and valuable imaging information for the diagnosis and differential diagnosis of pancreaticobiliary duct disease. The technique provides a convenient, reliable and quick method for two-dimensional image postprocessing for the imaging of pancreaticobiliary diseases.
