上颌快速扩弓联合前方牵引对骨性Ⅲ类错畸形上气道间隙及舌骨位置的影响

摘　　要：目的:探讨上颌快速扩弓联合前方牵引对骨性Ⅲ类错畸形上气道间隙及舌骨位置的影响。方法:22例青少年骨性Ⅲ错畸形患者,采用上颌快速扩弓联合前方牵引矫治6~13个月,比较矫治前后上气道间隙矢状径大小及颌骨、舌骨位置变化。结果:22例患者前牙反解除,<U1-SN减小(5.20±6.74)°,U1向前移动(6.84±9.32)mm,A向前移动(3.51±6.93)mm,Me向下移动(7.46±18.82)mm;H向下移动(5.17±7.78)mm,H与C3距离增大(2.53±3.67)mm;治疗后PNS平面气道矢状径增大(1.78±3.20)mm,C2平面气道矢状径增大(2.56±4.68)mm,差异有统计学意义(P<0.05);软腭处、C3平面气道矢状径均有增大,但差异无统计学意义(P>0.05)。结论:对有生长发育潜力的青少年患者,上颌快速扩弓联合前方牵引可增大鼻咽部及口咽部的矢状间隙,对气道下间隙影响不大。Objective： To evaluate the influence of protraction headgear with rapid maxillary expansion（ PE） on the upper airway space of skeletal Class Ⅲ malocclusion and hyoid position. Methods：22 cases with skeletal Class Ⅲ malocclusion received treatment of PE for 6 ~ 13 months. Cephalometric radiographs were obtained before（ T0） and after treatment（ T1）,and were analyzed. Results： After PE treatment,the anterior cross bite was corrected, U1-SN was reduced to 5. 20° ± 6. 74°,and the subspinale（ A） of maxillary and upper incisor（ U1） moved in the anterior direction. The Menton（ Me） of mandible and hyoid bone moved in the inferior direction. The distance of hyoid and the third cervical vertebra was increased to（ 2. 53 ± 3. 67） mm. The upper airway sagittal dimensions of hard palate and the second cervical increased. The difference was statistically significant（ P〈0. 05）. The upper airway sagittal dimensions of the third cervical vertebra and soft palate increased,but the difference was not statistically significant（ P〈0. 05）. Conclusion： For growing-up young patients,protraction headgear with rapid maxillary expansion（ PE） can increase the upper airway sagittal dimen-sions of nasopharynx and oropharynx,but has little effect on the lower airway space.
