显微镜下病灶切除联合结膜瓣遮盖术治疗难治性角膜溃疡的临床效果分析

摘　　要：目的探讨显微镜下病灶切除联合结膜瓣遮盖术治疗难治性角膜溃疡的临床效果。方法将2015年4月~2017年4月在我院治疗的60例难治性角膜溃疡患者随机分为对照组和观察组,各30例。对照组采用常规药物治疗,观察组采用显微镜下病灶切除联合结膜瓣遮盖术治疗,比较两组患者的临床疗效、并发症发生率、视力状况。结果观察组的治愈率为93.33%,明显高于对照组的66.67%,差异有统计学意义(P<0.05)。观察组继发青光眼、角膜穿孔、角膜葡萄肿、结膜瓣部分溶解、结膜瓣穿孔、结膜瓣滑脱等的并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。观察组治疗后的视力水平明显高于对照组,差异有统计学意义(P<0.05)。结论显微镜下病灶切除联合结膜瓣遮盖术治疗难治性角膜溃疡的临床效果确切,能有效保存眼球,促进视力的提高,且并发症发生率低,安全性高,具有积极的临床意义。Objective To investigate the clinical effect of microscopic resection combined with conjunctival flap surgery in the treatment of refractory corneal ulcer.Methods 60 cases of refractory corneal ulcer from April 2015 to April 2017 in our hospital were randomly divided into the control group and the observation group,30 cases in each group.The control group was treated with routine medication,the observation group was treated with microscopic resection combined with conjunctival flap surgery.The clinical efficacy,the incidence rate of complications and visual acuity between the two groups were compared.Results The cure rate of the observation group was 93.33%,which was higher than that of the control group （66.67%）,the difference was statistically significant （P〈0.05）.The incidence rate of secondary glaucoma,corneal perforation,corneal staphyloma,conjunctival flap partial dissolution,conjunctival flap perforation and conjunctival flap slippage in the observation group was lower than that in the control group,the difference was statistically significant （P〈0.05）.The visual acuity of the observation group after treatment was higher than that of the control group,the difference was statistically significant （P〈0.05）.Conelusion The clinical effect of microscopic resection combined with conjunctival flap surgery in the treatment of refractory corneal ulcer is significant,which can effectively save the eye, promote the improvement of visual acuity,low complication rate and high safety,it has positive clinical significance.
