原发性结直肠癌1092例5年回顾性分析

摘　　要：目的:探讨近5年原发性结直肠癌的临床流行病学特点。方法:收集2012年1月月至2016年12月近5年经内镜确诊并住院治疗的原发性结直肠癌患者的临床资料,并按确诊时的年龄分组,分析其临床表现、病变部位、病理类型、基础疾病、转移发生率及并发症等特点。结果:(1)原发性结直肠癌患者的中位诊断年龄为65.8岁,发病的高峰年龄为70~80岁;(2)左半结肠癌占71.0%;(3)病理组织类型中腺癌占93.4%,中分化腺癌占94.5%;(4)不同年龄组中,腹痛、体重下降、腹泻的发生率差异有统计学意义(P<0.05);(5)不同部位结直肠癌中,腹痛、血便、腹泻、贫血、腹胀、腹部包块的发生率差异均有统计学意义(P<0.05);(6)不同部位结直肠癌中肿瘤转移发生率及并发症发生率差异有统计学意义(P<0.01,P<0.05)。结论:结直肠癌的常见症状仍然是腹痛、血便、排便习惯改变、体重下降、腹胀、腹泻、贫血及腹部包块等;最常见的病理类型是腺癌;结直肠癌的好发部位是左半结肠,但右半结肠的比例有增多趋势;结直肠癌常见的合并症是胆囊结石;右半结肠癌的转移及肠梗阻发生率均较高。Objective：To investigate the clinical epidemiological characteristics of primary colorectal cancer in recent 5 years. Methods：The clinical data of patients with primary colorectal cancer,who were diagnosed by endoscopy and hospitalized in recent 5 years, between January 2012 and December 2016, were included and divided into groups according to the age at the diagnosis. The characteristics,such as the clinical manifestations, lesions,pathological types,underlying diseases,metastasis rate and complications were analyzed. Results：（1） The median age of primary colorectal cancer was 65.8 years old,and the peak age of onset was 70~80 years old. （2） The left colon cancer accounted for 71.0%. （3） The pathological type of adenocarcinoma accounted for 93.4%,and the sub-differentiation of adenocarcinoma accounted for 94. 5%. （ 4 ） There were statistically significant differences in the incidences of abdominal pain,weight loss and diarrhea among different age groups （P〈0.05）. （5） There were statistically significant differences in the incidences of abdominal pain,bloody stool, diarrhea,anemia,abdominal distension and abdominal mass at different parts of colorectal cancer （P〈0.05）. （6） There were statistically significant differences in the incidences of tumor metastasis and complications at different parts of colorectal cancer （P〈0.05）. Conclusion：（1） The common symptoms of colorectal cancer are still abdominal pain, bloody stool, change of bowel habits, weight loss, abdominal distension, diarrhea, anemia and abdominal mass. （ 2 ） The most common pathological type is adenocarcinoma. （ 3 ） The left colon is the predilection site of colorectal cancer,but the proportion of the right colon tends to increase. （ 4） The common complication of colorectal cancer is gallstone. （ 5） The occurrence of the right colon cancer metastasis and the intestinal obstruction are high.
