成釉细胞瘤P63、Bcl-2、PCNA、Ki-67、MCM2的表达及意义

摘　　要：目的探讨P63、Bcl-2、PCNA、Ki-67、MCMC2基因蛋白表达与成釉细胞瘤生物学行为的关系。方法60例成釉细胞瘤患者在河北医科大学口腔医院接受手术治疗。手术标本的存档蜡块及10例正常口腔黏膜标本,采用PV-9000免疫组化方法检测抗凋亡蛋白P63、Bcl-2,肿瘤增殖抗原PCNA、Ki-67、MCM2蛋白在成釉细胞瘤滤泡型和丛状型中的表达情况,进行定性及定量分析,并结合肿瘤包膜侵犯及临床复发情况进行探讨。结果 (1)P63的阳性表达在成釉细胞瘤滤泡型中低于丛状型(P<0.05),在正常组中的表达与滤泡型之间差异有显著性。P63在肿瘤的包膜是否侵犯病例及是否发生术后复发病例方面均具有显著性差异。(2)Bcl-2和PCNA在成釉细胞瘤滤泡型与丛状型中的表达分别与正常组之间差异有显著性。(3)成釉细胞瘤滤泡型中Ki-67的增殖指数和MCM2的阳性率均高于其在丛状型中的表达(P<0.05),且两型与正常组之间差异均有显著性。Ki-67和MCM2分别在术后复发病例与未复发病例之间比较差异有显著性。结论 (1)成釉细胞瘤P63的低表达与肿瘤包膜侵犯及复发密切相关。(2)成釉细胞瘤Ki-67和MCM2的高表达与肿瘤的复发密切相关。Objective To investigate the relationship between the expression of P63, Bcl-2, PCNA, Ki-67 and MCMC2 gene and the biological behavior of ameloblastoma. Methods Sixty patients with ameloblastoma were enrolled from the hospital of Stomatology, Hebei Medical University who received surgical treatments. The 60 archived ameloblastoma paraffin blocks and 10 cases of normal oral mucosa were detected by PV-9000 immunohistochemistry method against P63, Bcl-2, PCNA, Ki-67 and MCM2 in the follicular and plexiform patterns of ameloblastoma. The data were analyzed qualitatively and quantitatively combined with tumor capsule invasion and clinical recurrence. Results（1）The positive expression of P63 in plexiform pattern was lower than that in the follicular pattern of ameloblastoma（P0.05）. The positive expression of P63 in the normal group was significantly different from that in the follicular pattern of ameloblastoma. P63 has significant difference in the case of tumor capsule invasion and the occurrence of postoperative recurrence.（2） The expressions of Bcl-2 and PCNA in the follicular and plexiform pattern of ameloblastoma were significantly different from those of the normal group.（3）The proliferation index of Ki-67 and the positive rate of MCM2 in follicular pattern of ameloblastoma were higher than that in plexiform pattern（P 0.05）, and there was significant difference between the normal group and the two groups. Ki-67 and MCM2 were significantly different between the cases with recurrence and those without recurrence. Conclusion（1） The low expression of P63 is closely related to the invasion and recurrence of tumor capsule.（2）The high expression of Ki-67 and MCM2 is closely related to tumor recurrence.
