摘　　要：目的研究肉毒毒素(BTXA)注射联合平衡阴阳针刺法治疗脑外伤后上肢肌痉挛的临床疗效。方法选择76例脑外伤后上肢肌痉挛患者,随机分为观察组和对照组。两组均予常规对症治疗及肢体运动康复疗法。观察组注射肉毒毒素加用平衡阴阳针刺法。对照组予普通针刺法。比较注射前和注射后1月、2月、3月的改良Ashworth肌痉挛量表(MAS)、Barthel指数、Fugl-Meyer运动功能评测(FMA)评定情况。结果治疗后1月、2月、3月观察组与对照组各评定结果比较,差异有统计学意义(P<0.05),治疗后1月、2月、3月观察组与本组治疗前比较差异有统计学意义(P<0.01)。结论肉毒毒素注射联合平衡阴阳针刺法可缓解脑外伤患者上肢肌痉挛症状,提高运动协调性,积极配合系统康复训练,可有效改善上肢肢体运动功能,提高日常生活能力。Objective To study the clinical effect of botulinum toxin A（BTXA） injection combined with yin-yang balance acupuncture therapy on upper limb spasticity after traumatic brain injury.Methods 76 patients with upper limb spasticity after brain trauma were randomly divided into observation group and reference group.Two groups were given conventional symptomatic treatment and physical rehabilitation exercise.The observation group was injected with botulinum toxin and added balance acupuncture.The control group was given ordinary acupuncture.The modified Ashworth spasticity scale（MAS）,Barthel index and Fugl-Meyer motor function evaluation（FMA） were compared before and after injection1 month,2 months and 3 months.Results After treatment 1 month,2 months and 3 months,the evaluation results of the observation group and the control group were compared,the difference was significant（P〈0.05）,after treatment 1 month,2 months and 3 months,the evaluation results of the observation group has more significance than pretherapy（P〈0.01）.Conclusion Botulinum toxin injection combined with acupuncture can relieve the balance of yin-yang of patients with upper limb spasticity cerebral trauma symptoms,improve motor coordination,actively cooperate with the rehabilitation training,can effectively improve the upper limb motor function,improve the ability of daily life.
