额叶变异型阿尔茨海默病的临床及影像学特征

摘　　要：目的探索额叶变异型阿尔茨海默病(fvAD)的临床及影像学特点。方法分析2例在天津市环湖医院痴呆诊疗中心确诊的fvAD患者的临床表现、辅助检查及影像学资料。结果患者以认知功能障碍和精神行为异常起病,认知功能障碍累及包括记忆、语言、执行功能等多个认知领域,随着病情的进展,精神行为异常的表现越来越突出。^(18)F-FDG PET显示双额内外侧、颞叶外侧、顶叶、岛叶、楔前叶及扣带回、双尾状核、左丘脑代谢减低。^(11)C-PiB PET显示大脑皮层Aβ沉积。结论 fvAD与额颞叶痴呆的临床表现相似,PiB-PET检查有助于两者的鉴别。Objective To explore the clinical and neuroimaging features of frontal variant Alzheimer＇s disease（fv AD）. Methods We collected and analyzed the clinical manifestations,auxiliary examinations and neuroimaging data of 2 patients with fv AD diagnosed at the dementia center in Tianjin Huanhu Hospital. Results The patients present cognitive dysfunction and behavioral abnormalities including memory,language,executive function and many other areas. With the progression of the disease,the manifestations of mental and behavioral disorders become more and more prominent.18 F-FDG PET showed reductions in both the medial and lateral lobes,lateral temporal lobes,parietal lobes,insula,anterior and posterior cingulate gyrus,double caudate nucleus,and left thalamus. 11 C-Pi B PET showed deposition of A beta in the cerebral cortex. Conclusion The clinical manifestations of fv AD may be the same with frontal temporal dementia,Pi B-PET examination is helpful to differentiate these diseases.
