摘　　要：目的研究癌胚抗原(CEA)、CA15-3、酒石酸酸性磷酸酶(TRACP)结合全身骨显像对老年乳腺癌骨转移的诊断价值。方法选取我院2014年7月到2016年4月收治的老年乳腺癌病人106例为研究对象,对所有病人进行SPECT全身骨显像检查,并检测CEA、CA15-3及TRACP等指标,同时分析各指标诊断的准确度。结果共67例(63.21%)病人发生骨转移。随着Soloway分级增加,乳腺癌病人各血清肿瘤标志物水平均明显升高(F=66.28,54.29,42.84;P均<0.05)。3种肿瘤标志物联合诊断的准确度均高于单独诊断(P<0.05),SPECT骨显像联合3种标志物诊断的准确度高于3种肿瘤标志物联合诊断(P<0.05)。结论 SPECT全身骨显像联合各肿瘤标志物水平检查可明显提高老年乳腺癌骨转移的诊断准确度。Objective To study the diagnosis values of carcinoembryonic antigen（ CEA）,CA15-3,tartrate resistant acid phosphatase（ TRACP） combined with whole body bone imaging in bone metastasis in elderly patients with breast cancer. Methods 106 elderly patients with breast cancer in our hospital from July 2014 to April 2016 were selected. All patients received SPECT bone imaging and detection of CEA,CA15-3 and TRACP. The diagnostic accuracy of all approaches were calculated and compared. Results A total of 67（ 63. 21%） cases presented with bone metastasis. With the increase of Soloway grade,the levels of CEA,CA15-3 and TRACP were significantly increased（ F = 66. 28,54. 29,42. 84; P〈0. 05）. The diagnostic accuracy of three tumor markers combined was higher than that of single marker（ P〈0. 05）; the diagnosis accuracy of the SPECT bone imaging combined with 3 markers was higher than the three tumor markers combined diagnosis（ P〈0. 05）. Conclusions SPECT bone imaging combined with tumor markers examination can significantly improve the diagnostic accuracy of bone metastasis in elderly patients with breast cancer.
