术前新辅助化疗联合宫颈癌根治术对Ⅰb2~Ⅱb期宫颈癌患者疗效及预后的影响研究

摘　　要：目的探究术前新辅助化疗联合宫颈癌根治术对Ⅰb2~Ⅱb期宫颈癌患者的治疗效果以及预后的影响。方法从2015年2月~2017年5月在我院收治的Ⅰb2~Ⅱb期宫颈癌患者164例随机抽取82例作为研究组,剩余82例作为对照组,对照组使用宫颈癌根治术进行治疗,研究组使用术前新辅助化疗联合宫颈癌根治术进行治疗。在治疗完成后,比较两组患者的治疗效果,手术时间、术中出血量、膀胱功能恢复时间以及术后住院时间,术后病理状况。结果对照组的有效率为60.08%;研究组的有效率为79.27%;研究组患者的手术时间缩短,术中出血量降低,膀胱功能恢复时间以及术后住院时间均有明显的降低;研究组患者淋巴结转移、阴道切缘残留、无癌细胞、宫颈深肌层浸润、宫旁浸润等现象的发生率较低,低于对照组,具有统计学意义(P<0.05)。结论在Ⅰb2~Ⅱb期宫颈癌患者的治疗过程中,采用术前新辅助化疗联合宫颈癌根治术的方式进行治疗的效果显著,安全可靠,为以后深入研究和应用于临床提供了参考依据。Objective To investigate the effect of preoperative neoadjuvant chemotherapy combined with radical resection of cervical cancer on the curative effect and prognosis of patients with stage Ⅰb2 toⅡb cervical cancer. Methods 164 cases with Ⅰb2- Ⅱb cervical cancer from February 2015 to May 2017 in our hospitalwere selected and randomly divided 82 cases into the study group,the remaining 82 cases as the control group.The control group was given cervical cancer radical surgery,the study group was given preoperative neoadjuvant chemotherapy combined with cervical cancer radical surgery.After treatment,the treatment effect,operation time,intraoperative blood loss,recovery time of bladder function and postoperative hospital stay and postoperative pathological condition were compared between the two groups. Results The effective rate of the control group was 60.08%;the effective rate of the study group was 79.27%.In the study group,the operation time was shortened,the intraoperative blood loss was reduced,the bladder function recovery time and postoperative hospital stay were significantly reduced;The incidence of lymph node metastasis,residual vaginal margin,no cancer cells,deep myometrial invasion,and para uterine infiltration in the study group was lower than that in the control group,P 〈 0.05,which was statistically significant. Conclusion Effect of preoperative neoadjuvant chemotherapy combined with radical hysterectomy for cervical cancerin the treatmentof patients with stage Ⅰb2 to Ⅱ h cervical canceris remarkable,safe and reliable.It provides a reference for further research and clinical application.
