摘　　要：目的回顾性分析比较重组组织型纤溶酶原激活剂(rt-PA)与尿激酶(UK)静脉溶栓治疗急性脑梗死疗效。方法回顾性分析2016年1月~2017年6月我院急诊科收治的80例急性脑梗死患者临床资料,根据不同溶栓药物将其分为rt-PA组(40例)和UK组(40例),比较两组患者溶栓前与溶栓后2h心率(HR)、平均动脉压(MAP)水平,治疗前、溶栓后24h和7d时NIHSS评分,不良反应发生情况。结果 (1)rt-PA组与UK组患者溶栓前、溶栓后2h HR和MAP水平差异均无统计学意义(P>0.05)。(2)rt-PA组溶栓前、溶栓后24h和7d时与UK组NIHSS评分差异均无统计学意义(P>0.05),溶栓后24h NIHSS评分下降≥4分例数和溶栓后7d NIHSS评分下降≥4分患者例数与UK组比较差异均无统计学意义(P>0.05)。(3)rt-PA组与UK组患者不良反应无显著差异。结论 rt-PA与UK静脉溶栓治疗急性脑梗死效果均较好。Objective To retrospectively analyze and compare the curative effect of recombinant tissue plasminogen activator （rt-PA） and nrokinase （UK） intravenous thrombolytic therapies on acute cerebral infarction. Methods The clinical data of 80 patients with acute cerebral infarction treated in emergency department of our hospital from January 2016 to June 2017 were retrospectively analyzed. According to different thrombolytic drugs,they were divided into rt-PA group （40 cases） and UK group （40 cases）.The heart rates （HR） and mean arterial pressure （MAP） before thrombolytic therapy and at 2h after thrombolytic therapy,NIHSS scores before treatment,at 24h and 7d after thrombolytic therapy and the incidence of adverse reactions were compared between the two groups. Results （1）There was no significant difference in the levels of HR and MAP before and at 2h after thrombolytic therapy between group rt-PA and group UK （P 〉 0.05）.（2）There was no significant difference in NIHSS scores between the two groups before thrombolytic therapy,and at 24h and 7d after thrombolytic therapy （P - 0.05）.There was no significant difference between the two groups in the number of patients whose decreased score of NIHSS score ≥ 4 points （P 〉 0.05）.（3） There was no significant difference in adverse reactions between the two groups. Conclusion The effects of rt-PA and UK intravenous thrombolytic therapies are good in the treatment of acute cerebral infarction.
