摘　　要：目的探究i-scan成像技术与碘染色对Barrett食管诊断价值。方法选取2016年1月～2017年7月由于各种上消化道症状至我院行内镜检查而疑诊为BE的患者120例作为研究对象,按照随机分配的原则将其分为碘染色组﹑i-scan成像组及对照组,各40例。三组患者分别采用碘染色﹑i-scan成像和常规内镜随机4象限活检,比较三组患者的病理符合率及BE中肠化﹑不典型增生和早期腺癌的检出率。结果三组患者病理符合率的差异有统计学意义(P<0.05),碘染色组和i-scan成像组的病理符合率均显著高于对照组(P<0.05),但碘染色组和i-scan成像组的病理符合率的比较并无统计学意义(P>0.05);三组患者BE中肠化﹑不典型增生和早期腺癌检出率的比较均无统计学意义(P>0.05)。结论 i-scan成像技术与碘染色同随机4象限活检相比,具有较高的病理符合率,对于BE的诊断价值大,但是对于BE食管中肠化﹑不典型增生和早期腺癌的检出率的提升并没有意义,尚需要更进一步的探索和研究。Objective To explore the diagnostic value of I-Scan imaging technique and iodine staining in Barrett＇s esophagus. Methods 120 cases of patients underwent endoscopic examination and suspected BE due to various symptoms of upper digestive tract in our hospital from January 2016 to July 2017 were chosen as the research object,and divided into iodine staining group,I-Scan imaging group and control group,according to the principle of random distribution,40 cases in each group.Patients in the three groups were treated with iodine staining,I-Scan imaging and routine endoscopic random 4 quadrant biopsy separately.The pathological coincidence rate and the detection rate of BE midgut hyperplasia,atypical hyperplasia and early adenocarcinoma were compared between the three groups. Results The difference of pathological coincidence rate between the three groups was statistically significant（P - 0.05）.The pathological coincidence rate of iodine staining group and I-Scan imaging group was significantly higher than that of control group（P 〈 0.05）,but there was no significant difference in the coincidence rate between iodine staining group and I-Scan Imaging Group（P 〉 0.05）.There was no significant difference in the detection rate of BE between midgut,atypical hyperplasia and early adenocarcinoma in the three groups （P 〉 0.05）. Conclusion I-Scan imaging technique and iodine staining have higher pathological coincidence rate compared with random 4 quadrant biopsy,and is of great value in the diagnosis of BE,however,there is no significance in enhancing the detection rate of intestinal metaplasia,atypical hyperplasia and early adenocarcinoma in BE,still need further exploration and research.
