血清幽门螺杆菌抗体分型联合^(14)C-尿素呼气试验检测幽门螺杆菌感染的临床应用

摘　　要：目的探讨血清幽门螺杆菌抗体分型联合^(14)C-尿素呼气试验(^(14)C urea breath test,^(14)C-UBT)在幽门螺杆菌(Helicobacter pylori,Hp)感染中的应用价值。方法选择2016年10月—2017年7月九师医院门诊、住院行幽门螺杆菌抗体分型检测联合尿素^(14)C-UBT测定的522例患者作为研究对象,分析两种方法联合检测的临床意义。结果^(14)C-UBT与血清幽门螺杆菌抗体分型检测Hp阳性率分别为68.4%和64.2%,差异无统计学意义(P>0.05)。^(14)C-UBT与血清幽门螺杆菌抗体分型检测阳性符合率为82.4%(294/357)、阴性符合率为75.15%(124/165),总符合率为80.08%。357例^(14)C-UBT阳性者中,血清抗体Ⅰ型209例,Ⅱ型85例,阴性63例。结论血清幽门螺杆菌抗体分型联合^(14)C-UBT能够准确筛查Hp感染者并进行分型,对临床抗生素治疗具有重要指导作用。Objective: To investigate the value of Serum Helicobacter pylori antibody typing combined with ^(14)C-urea breath test( ^(14)C-UBT) in Helicobacter pylori( Hp) infection. Methods: 522 patients in out-and in-patient of Jiushi Hospital from October 2016 to 2017 were selected and given Helicobacter pylori antibody typing combined with urea ^(14)C-UBT. The clinical significance of combined detection of two kinds of methods was analyzed. Results: The positive rate of Hp for ^(14)C-UBT and antibody typing test were 68. 4% and 64. 2%,respectively,and the difference was not statistically significant( P >0. 05). The positive coincidence rate of ^(14)C-UBT and serum antibody typing was 82. 4%( 294/357),and the negative coincidence rate was 75. 15%( 124/165),and the total coincidence rate was 80. 08%. Among the 357 cases of ^(14)C-UBT positive,there were 209 cases of serum antibody type I,85 cases of type II,and 63 cases of negative. Conclusion: The Serum Helicobacter pylori antibody typing combined with ^(14)C-UBT can accurately screen Hp infected persons and classify them,which plays an important guiding role in clinical antibiotic treatment.
