摘　　要：目的探讨肝动脉化疗栓塞(TACE)对原发性肝癌患者自然杀伤细胞及淋巴细胞亚群的变化影响。方法回顾性分析2013年1月—2015年12月在江苏省肿瘤医院介入科接受TACE治疗的20例原发性肝癌患者的临床资料。流式细胞仪检测患者治疗前、治疗后7天、14天及30天时外周血中NK细胞、CD3^+、CD4^+、CD8^+及CD4^+/CD8^+比值的动态变化。结果TACE治疗后7天,患者外周血CD4^+、CD4^+/CD8^+比值均下降,CD8^+细胞比率上升,且较TACE前具有统计学意义(P<0.05)。TACE治疗后14天,患者外周血NK、CD4^+细胞的比率及CD4^+/CD8^+比值继续下降,CD8^+细胞比率上升,较TACE前差异均有统计学意义(P<0.05)。TACE后30天,患者外周血NK、CD4^+细胞及CD4^+/CD8^+比率均有所恢复,而CD3^+细胞继续减少,其中CD4^+及CD3^+细胞与TACE治疗前相比,减少有统计学意义(P<0.05)。结论TACE治疗原发性肝癌可造成患者一过性细胞免疫功能抑制状态,但可逐渐恢复。Objective To investigate the effect of transcatheter arterial chemoembolization（TACE）on natural killer cell and lymphocyte subsets in patients with primary liver cancer.Methods The clinical data of 20patients with primary liver cancer treated with TACE from January 2013 to December 2015 in the interventional department of Jiangsu tumor hospital were analyzed retrospectively.The dynamic changes of NK cells,CD3^＋,CD4^＋,CD8^＋and CD4^＋/CD8^＋ratio in peripheral blood of patients before and after 7 days,14 days and 30 days were detected by flow cytometry.Results On the 7 day after TACE treatment,the peripheral blood CD4^＋,CD4^＋/CD8^＋ratio decreased,and the ratio of CD8^＋cells increased,and there was statistically significant difference compared with that before TACE（P〈0.05）.On the 14 day after TACE treatment,the ratio of peripheral blood NK and CD4^＋cells and the ratio of CD4^＋/CD8^＋continued to decline,and the ratio of CD8^＋cells increased,which was statistically different from that before TACE（P〈0.05）.On the 30 day after TACE,peripheral blood NK,CD4^＋cells and CD4^＋/CD8^＋ratio recovered,while CD3^＋cells continued to decrease,and CD4^＋and CD3^＋cells decreased significantly compared with those before TACE treatment（P〈0.05）.Conclusion The treatment of primary liver cancer with TACE can lead to a transient inhibition of cellular immune function,but can gradually recover.
