超声骨刀在口腔种植上颌窦提升术中的应用效果评价

摘　　要：目的 观察超声骨刀在口腔种植上颌窦提升术中的应用效果，以期为口腔种植上颌窦提供新 的选择方法。方法 选择８０例上颌后牙缺失患者为研究对象，按照随机数字表法分为观察组和对照组，每组 ４０例。观察组采用超声骨刀进行上颌窦提升术，对照组则采用经牙槽嵴入路冲顶法进行上颌窦提升术。比 较两组行上颌窦提升术后口腔种植效果，并采用疼痛视觉模拟量表（ＶｉｓｕａｌＡｎａｌｏｇｕｅＳｃｏｒｅ，ＶＡＳ）对两组患者 术后的疼痛情况进行评分比较，采用自制调查表对两组患者口腔种植修复的满意度进行比较，对两组行提升 术后的上颌窦底提升高度、种植体边缘骨吸收量进行比较。结果 两组上颌窦提升术后，口腔种植体稳固无 脱落，经过３个月的随访，观察组有２例出现种植体脱落，对照组出现３例脱落，组间差异无统计学意义（χ２＝ ０．２１３，Ｐ＝０．６４４）；观察组术后１ｈ、６ｈ的ＶＡＳ评分显著低于对照组，差异均有统计学意义（ｔ＝２．１２４、３．８００， Ｐ＝０．００５、０．０００）；观察组在种植修复后在美观功能、舒适度、咀嚼功能、发音功能等四项功能的评分显著高 于对照组，差异均有统计学意义（ｔ＝４．８８９、３．８７２、２．４１１、３．３９７，Ｐ＝０．００１、０．００１、０．０１８、０．００１）；观察组上颌 窦底提升高度、种植体边缘骨吸收量均高于对照组，差异有统计学意义（ｔ＝５．８１０、８．００６，Ｐ＝０．０００、０．０００）。 结论 超声骨刀在口腔种植上颌窦提升术中的应用有较好的临床效果，能够降低患者的疼痛感，提高患者对 口腔种植体的满意度，有较高的临床应用价值。Objective Tostudytheeffectoftheapplicationoftheultrasonicscalpelinthemaxillarysinus augmentationoforalcavity.Methods 80casesofmaxillaryposteriorteethmissingpatientswereselectedasthe researchsubjects.Thepatientswererandomlydividedintoobservationgroupandcontrolgroupaccordingtothedigital table,40casesineachgroup.Thepatientsintheobservationgroupweretreatedwithmaxillarysinuselevationby ultrasonicscalpel,whilethecontrolgroupwastreatedwiththemethodofthealveolarridgeapproach.Theoralimplant effectaftermaxillarysinusaugmentationwascomparedbetweenthetwogroups,andthepainvisualanaloguescore （VAS）wasusedtocomparepostoperativepainbetweenthetwogroups,thesatisfactionofthetwogroupsofpatients withoralimplantrepairwascomparedbytheself-madequestionnaire,theelevationofmaxillarysinusfloorelevation andtheamountofmarginalboneresorptionwerecomparedbetweenthetwogroups.Results Thetwogroupswith maxillarysinusaugmentationimplantedfirmlywithoutfallingoff,after3monthsoffollow-up,2patientsofthe observationgrouphadimplantfailure,3casesoflossinthecontrolgroup,thedifferencebetweenthetwogroupswas notstatisticallysignificant（χ2 =0.213,P=0.644）.TheVASscoreoftheobservationgroup1hand6hafter operationwassignificantlylowerthanthatofthecontrolgroup,thedifferencewasstatisticallysignificant（t=2.124, 3800,P=0.005,0.000）.Intheobservationgroup,thescoresofthefixedfunction,aestheticfunction,comfort, chewingfunctionandpronunciationfunctionweresignificantlyhigherthanthoseofthecontrolgroupafterthe restoration,andthedifferenceswerestatisticallysignificant（t=4.889,3.872,2.411,3.397,P=0.001,0.001, 0018,0.001）.Intheobservationgroup,theelevationofthemaxillarysinusfloorelevationandtheamountofthe marginalboneresorptionwerehigherthanthoseofthecontrolgroup,thedifferenceswerestatisticallysignificant（t= 5.810,8.006,P=0.000,0.000）.Conclusion Ultrasoundboneknifeinoralimplantmaxillarysinushasbetter clinicaleffecttoenhancetheclinicalapplication,itcanreducethepainofpatients,improvepatients＇satisfactionof dentalimplant,ithashigherclin
