MSCT诊断直肠癌直肠系膜浸润、环周切缘状态的价值及其与病理特征的关系

摘　　要：目的探讨多层螺旋CT(Multi-slice spiral CT,MSCT)预测直肠癌直肠系膜浸润、环周切缘状态的临床意义,并观察其与临床病理特征的关系。方法回顾性分析本院2014年1月至2016年6月收治的92例直肠癌患者临床资料,均经病理证实直肠癌,所有患者术前均进行MSCT扫描,判断患者直肠系膜浸润程度与环周切缘状态,与病理结果对照诊断符合率,并探究其判断结果与临床病理特征的关系。结果以病理结果为参照,MSCT预测直肠系膜浸润的符合率为94.57%(87/92),MSCT测量环周切缘状态的符合率为95.65%(88/92),与病理结果比较,差异均无统计学意义(均P>0.05)。MSCT预测直肠系膜浸润、环周切缘状态均与患者肿瘤直径、T分期及是否发生淋巴结转移及有关(均P<0.05)。结论 MSCT预测直肠系膜浸润、环周切缘状态价值较高,且其判断结果其与患者肿瘤直径、T分期及是否发生淋巴结转移有关。Objective To investigate the clinical value of multi-slice spiral CT（MSCT） in diagnosing rectal carcinoma with mesorectal invasion and circumferential resection margin, and to determine its relationship with pathological features. Methods We performed a retrospective analysis of 92 patients with rectal cancer from January 2014 to June 2016 in our hospital. All patients were pathologically confirmed with rectal cancer, underwent MSCT scan. The degree of tumor infiltration in mesorectum, circumferential resection margin status and diagnosis rate were evaluated, and the relationship between the MSCT findings and clinical pathological parameters.Results With pathological findings as reference, diagnostic consistence rate of infiltration in mesorectum for MSCT was 94.57%（87/92）, consistence rate for measurement of circumferential resection margin status was 95.65%（88/92）. There was no significant difference compared with the pathological findings（P〉0.05）. MSCT predicted invasion of the mesorectal membrane and the circumferential margin of resection were related to tumor diameter, T staging and lymph node metastasis（P〈0.05）. Conclusion MSCT predicted tumor invasion, circumferential resection and margin resection, which was related to tumor diameter, T staging and lymph node metastasis.
