摘 要:目的探讨纺锤体组装检查点蛋白Mad1在原发性胃肠淋巴瘤(PGIL)中的表达及意义。方法选取2004年1月至2010年12月间武汉大学中南医院收治的21例原发性胃肠淋巴瘤患者的手术切除或者内镜活组织标本,同时收取患者的10份正常胃肠组织、8份反应性增生淋巴结和15份结内非霍奇金淋巴瘤组织为对照。采用免疫组化法检测Mad1、Ki-67的表达及幽门螺杆菌(Hp)感染,采用荧光原位杂交(FISH)方法检测Bcl-6基因易位情况。结果 Mad1在原发性胃肠淋巴瘤中的表达,高于正常胃肠组织和反应性增生淋巴结,差异均有统计学意义(均P<0.05),与结内非霍奇金淋巴瘤表达差异无统计学意义(P>0.05),Mad1表达与Ki-67表达呈正相关,差异有统计学意义(P<0.05)。与Bcl-6基因易位、Hp感染、性别、年龄、临床分期及国际预后指数等无关,差异均无统计学意义(均P>0.05)。结论 Mad1在PGIL中表达的增高与Ki-67增殖指数呈正相关,可成为对PGIL进行诊断和评估临床预后的生物学标志。Objective To assess the expression of mitotic arrest deficiency protein 1( Mad1) in primary gastrointestinal lymphoma( PGIL) and its clinical significance. Methods Tissue specimens were collected by resection or endoscopic biopsy from 21 PGIL patients who treated at Zhongnan Hospital of Wuhan University between January 2004 and December 2010. Normal gastrointestinal tissues from 10 patients with cancer,reactive lymph nodes from 15 patients,nodal non-Hodgkin's lymphoma from 8 patients were collected as the control group. Immunohistochemistry was used to detect the expression Mad1 and Ki-67 and the infection of Helicobacter pylori( H. pylori). Fluorescent in situ hybridization( FISH) was used to detect the expression of Bcl-6. Results The expression of Mad1 was higher in PGIL than in normal gastrointestinal tissues and reactive lymph node proliferation( all P〈0. 05) but no statistical significant difference was noted compared with nodal non-Hodgkin's lymphoma( P〈0. 05). The expression of Mad1 was positively related to Ki-67( P〈0. 05) rather than with BCL-6 gene translocation,H. pylori infection,gender,age,clinical stage and IPI( all P〈0. 05). The median progression-free survival was longer in Mad1 higher expression group than in lower expression group( P〉0. 05). Conclusion Increasing expression of Mad1 in PGIL is closely related to Ki-67 index,which might be a promising biomarker for PGIL diagnosis and prognostic evaluation.
