四种肿瘤标志物检测在胃癌诊断中的应用

摘　　要：目的探讨血清肿瘤标志物癌胚抗原(CEA)、糖类抗原CA19-9、CA72-4及CA125在胃癌诊断中的应用价值。方法选取2014年10月至2016年11月间河北北方学院附属第一医院收治的200例胃癌患者为研究组,另选取同期180例健康体检人员为对照组。检测并比较两组人员的CEA、CA19-9、CA72-4和CA125水平。结果研究组患者CEA、CA19-9、CA72-4和CA125水平均明显高于对照组患者,差异均有统计学意义(均P<0.05)。研究组Ⅲ~Ⅳ期患者CEA、CA19-9、CA72-4和CA125水平均高于Ⅰ~Ⅱ期患者,差异均有统计学意义(均P<0.05)。研究组高、中分化患者的CEA、CA19-9、CA72-4和CA125水平均低于低分化者患者,差异均有统计学意义(均P<0.05)。研究组患者血清CEA、CA19-9、CA72-4和CA125及联检的阳性率均高于对照组,差异均有统计学意义(均P<0.05)。四种血清肿瘤标志物联合检测灵敏度和准确率均高于单一检测,差异均有统计学意义(均P<0.05)。结论血清肿瘤标志物CEA、CA19-9、CA72-4及CA125联合检测可有效提高临床诊断敏感性与准确率。Objective To study the significance of serum tumor markers carcino-embryonic antigen（ CEA）,carbohydrate antigen 19-9（ CA19-9）,CA72-4 and CA125 for the diagnosis of gastric cancer.Methods A total of 200 patients with gastric cancer who underwent treatment at First Affiliated Hospital of Hebei North University from October 2014 to November 2016 were selected as the study group and at the same period,another 180 healthy individuals who had physical examination were as the control group. The levels of CEA,CA19-9,CA72-4 and CA125 were detected. Results The levels of CEA,CA19-9,CA72-4 and CA125 were significantly higher in the study group than in the control group（ all P〈0. 05）.For the study group,the levels of CEA,CA19-9,CA72-4 and CA125 were significantly higher in patients with stage Ⅲ ~ Ⅳ gastric cancer than in patients with stage Ⅰ ~ Ⅱ gastric cancer（ all P〈0. 05）. For the study group,the levels of CEA,CA19-9,CA72-4 and CA125 were significantly lower in patients with high and moderate differentiation than patients with low differentiation（ all P〈0. 05）. The positive rate of serum CEA,CA19-9,CA72-4,CA125 and combined test was higher in the study group than in the control group（ all P〈0. 05）. The sensitivity and accuracy of the combined detection of the above four serum markers were higher than those of the single test（ all P〈0. 05）. Conclusion The combined detection of serum tumor markers CEA,CA19-9,CA72-4 and CA125 can effectively improve the sensitivity and accuracy of clinical diagnosis of gastric cancer.
