天丹通络、舍曲林联合治疗脑卒中后抑郁的疗效观察

摘　　要：目的观察天丹通络胶囊联合盐酸舍曲林治疗脑卒中后抑郁症(PSD)的临床疗效。方法 PSD患者64例,随机分为对照组和治疗组各32例。两组患者均给予脑卒中常规治疗,在此基础上对照组应用盐酸舍曲林50 mg,1次/d,口服;治疗组在对照组基础上加用天丹通络胶囊2.0 g,3次/d,口服。治疗2个月后观察两组患者临床疗效及抑郁程度、日常生活能力和神经功能缺损程度情况,分别采用汉密尔顿抑郁量表(HAMD)、日常生活量表(ADL)、中国卒中量表(CSS)评定。结果两组患者治疗后治疗组总有效率为92.75%,显著高于对照组的78.76%,两组比较差异有统计学意义(P<0.05)。两组治疗后HAMD、ADL、CSS评分均优于治疗前,差异有统计学意义(P<0.01),且治疗组优于对照组(P<0.05)。结论盐酸舍曲林配合天丹通络治疗PSD比单纯应用舍曲林效果显著,还可促进神经功能康复、提高日常生活自理能力。Objective To observe the clinical therapeutic effects on post-stroke depression treated with tiandantongluo capsules and sertraline. Methods A total of 64 patients of post-stroke depression were randomized into a control group and a treatment group,32 cases in each one. The routine treatment for stroke was given in the two groups. Additionally,in the control group,sertraline was applied,50 mg,once a day,oral administration. In the treatment group,on the basis of the treatment as the control group,tiandantongluo capsules was added,2. 0 g,three times a day,oral administration. The clinical therapeutic effects,depression degree（Hamilton depression scale,HAMD scale）,daily living activity（activities of daily living,ADL） and nerve function defect（Chinese stroke scale,CSS） were evaluated. Results The total effective rate was 92. 75% in the treatment group,remarkably higher than 78. 76% in the control group after treatment,indicating the significant difference（P〈0. 05）. After treatment,the scores of HAMD,ADL and CSS were all better than those before treatment in the two groups,indicating the significant differences（P〈0. 05）. The results in the treatment group were better than those in the control group（P〈0. 05）. Conclusion The combined medication of tiandantongluo capsules and sertraline achieves the remarkable effects on post-stroke depression as compared with the simple application of sertraline. It promotes the recovery of neural function and improves the daily living activity.
