摘 要:目的:探讨脑源性神经营养因子(BDNF)对重症抑郁患者抑郁症状及认知功能障碍的影响。方法:发作期重症抑郁患者111例为患者组,健康志愿者35例为对照组。分别采用自制的一般情况调查表、汉密尔顿抑郁量表(HAMD)-17和STROOP色词测验、言语流畅测验、逻辑记忆和视觉再生测验等工具评估一般情况、抑郁程度和认知功能。采集静脉血,抽提DNA,使用连接酶检测反应检测rs6265位点上的基因型,并检测血清BDNF水平。结果:患者组BDNF血清浓度低于对照组(P<0.05),患者组BDNF血清浓度与HAMD得分呈显著负相关(P<0.05),但与神经心理测验成绩无显著相关(P>0.05)。患者组按rs6265基因型分为AA、AG和GG3亚组,各亚组间神经心理测验成绩差异无统计学意义(P>0.05)。结论:血清BDNF水平与抑郁症状严重程度相关,对抑郁症患者认知功能无影响。Objective: The purpose of this study was to explore the relationship between the level of brain derived neurophic factor (BDNF) in serum and depressive symptoms and cognitive dysfunction of patients with major depression. Methods: One hundred and eleven patients with depression (depression group) and 35 healthy volunteers (control group) were enrolled. Self-made general questionnaire, Hamilton depression scale (HAMD)-17, Stroop neuropsychological screening test, word fluency test, logical memory and visual reproduc- tion test were used to assess the severity of depression and cognitive function. All subjects were genotyped with regard to BDNF polymorphisms (rs6265). The level of BDNF in serum was detected. Results: The level of BDNF in serum of depression group was significantly lower than that of the control group (P〈0.05). There was a negative correlation between the level of BDNF in serum and the HAMD test score (P〈0.05), but no significant relationship was detected between the level of BDNF in serum and the neuropsychological test score (P〉0.05). There was no significant relationship between BDNF polymorphisms and neuropsychological test scores (P〉 0.05). Conclusion: The level of BDNF in serum is related with depressive symptoms, but it has no effect on cognitive dysfunction in patients with major depression.
