摘　　要：目的研究甲状腺微小乳头状癌(papillary thyroid microcarcinoma,PTMC)不同年龄段的临床病理特点。方法 PTMC病人291例,根据病人年龄,将291例病人分为年龄<35岁组、35~44岁组和≥45岁组,分析不同年龄段病人的临床病理特点。结果初诊PTMC病人291例,占同期甲状腺癌病人的63.0%,年龄21~72岁,中位年龄46岁,男女比例1∶3.3;45岁以下病人占46.0%,男性病人比例低于女性(P<0.05)。随着年龄的增大,女性病人比例、选择双侧全切或近全切病人比例,及中央组淋巴结阴性比例均明显增加(P<0.05)。35岁以下病人中,除手术方式外,PTMC病人与非PTMC病人临床病理特点相似。结论 PTMC病人年龄不同,临床病理特点存在差异。应重视年轻、男性PTMC病人的筛查及风险评估。Objective To investigate the clinicopathological characteristics of the papillary thyroid microcarcinoma（PTMC） based on age.Methods A total of 291 cases with primary PTMC were enrolled in People＇s Hospital of Wuhan University from October 2015 to October 2016,and divided into three groups（ 35,35 - 44 and ≥45 years old）.The clinicopathological data were collected and analyzed based on age.Results The 291 cases with newly diagnosed PTMC accounted for 63% of the patients with thyroid cancer at the same period.The median age was 46 years old（21 - 72 years） and the ratio of male to female was 1∶ 3.3; the patients younger than 45 years old accounted for 46% and the proportion of male patients was significantly lower than that of female patients（P 0.05）.With the increasing of age,the proportion of female patients,bilateral total resection or subtotal resection,and the negative proportion of patients with lymph node were significantly increased（P 0.05）.In patients younger than 35 years old,the clinicopathological characteristics,except the surgical procedure,between the PTMC and non PTMC group were similar.Conclusions The clinicopathological characteristics of PTMC varied by the ages of patients.The screening and risk assessment of PTMC for young man should be paied much attention.
