大黄枳实汤对脑梗死急性期胃肠激素调节的作用研究

摘　　要：目的基于通腑法,观察大黄枳实汤对脑梗死急性期胃肠激素胃动素(MTL)、胃泌素(GAS)、血管活性肽(VIP)、生长抑素(SS)的变化以及大便症状积分的变化。方法将60例脑梗死急性期患者随机分为2组,2组均进行神经内科常规治疗。对照组30例在常规治疗的基础上加服枸橼酸莫沙必利分散片,治疗组30例在常规治疗的基础上加大黄枳实汤,2组均治疗7 d。比较2组治疗前后血清MTL、GAS、VIP、SS水平及大便症状积分变化。结果治疗后2组MTL、GAS、SS、VIP水平均较本组治疗前明显下降(P<0.05),但2组组间比较差异无统计学意义(P>0.05)。2组治疗后排便通畅度、便质、排便间隔时间评分均较本组治疗前明显下降(P<0.05),且治疗组低于对照组(P<0.05)。结论大黄枳实汤能够明显调节脑梗死急性期患者胃肠激素MTL、GAS、SS、VIP,改善患者胃肠功能。Objective To observe the changes of Dahuang-zhishi decoction on adjusting gastrointestinal hormones including motilin( MTL),gastrin( GAS),vasoactive intestine peptide( VIP) and somatostatin( SS) and the symptom score of defecation in cerebral infarction at acute phase. Methods 60 patients with cerebral infarction at acute phase were randomly divided into two groups,and the two groups were received neurology routine treatment.30 cases in the control group were treated by Mosapride citrate dispersible tablets on the basis of the routine treatment. 30 cases in the treatment group were treated by Dahuang-zhishi decoction on the basis of the routine treatment. The treatment course was 7 d in two groups. The levels of serum MTL,GAS,VIP,SS and the symptom score of defecation before and after treatment were compared in two groups. Results The levels of MTL,GAS,VIP and SS after treatment obviously reduced in two groups( P < 0. 05). There was no statistical difference on the MTL,GAS,VIP and SS after treatment between the two groups( P > 0. 05). The scores of defecation smoothness degree,defecation quality,defecation interval after treatment obviously reduced in two groups( P < 0. 05),and which in the treatment group were lower than that in the control group( P < 0. 05). Conclusion Dahuang-zhishi decoction can significantly regulate gastrointestinal hormones including MTL,GAS,SS and VIP of cerebral infarction patients at acute phase,and improve gastrointestinal function of patients.
