特制食管内^(125)I粒子链治疗晚期胸部恶性肿瘤导致进食困难临床研究

摘　　要：目的观察食管内^(125)I粒子链治疗晚期胸部恶性肿瘤导致食管梗阻引起进食困难的疗效。方法4例晚期胸部恶性肿瘤导致进食困难,分别为上段局部晚期食管癌1例,食管癌合并远处转移2例,肺癌巨大纵隔淋巴结转移压迫食管1例,术前均只能进全流食,并有间断呕吐,4例均不能手术治疗,行特制^(125)I粒子链食管内置入。结果 3例患者直接经鼻腔置入^(125)I粒子链并在上消化道泛影葡胺透视下调整在合适位置,1例在胃镜引导下置入粒子链,透视下将粒子链调整在合适位置。术后1~3周开始起效,进食困难逐渐缓解,术后45~60天拔除粒子链,上消化道透视显示梗阻解除。分别随访7、5、3和2个月,1例在7个月时再次出现进食梗噎感,其余3例进食正常。结论特制食管内^(125)I粒子链治疗晚期胸部恶性肿瘤导致食管梗阻引起的进食困难疗效显著,创伤小,有长时间维持进食通畅的可能。Objective To observe the curative effect of esophageal 125I particle chain in treat- ment of advanced thoracic malignant tumors leading to eating difficulties caused by esophageal obstruction. Methods Four cases of advanced thoracic malignant tumors with difficulty of eat- ing were performed line 125! particles of special esophageal implantation, the improvement of eating difficulties was observed. Results Three patients were directly implanted into the 125I particle chain in the nasal cavity and adjusted in the upper gastrointestinal tract, and 1 case was placed in the particle chain under the guidance of the endoscopy. The particle chain was adjusted to the proper position under fluoroscopy. One to three weeks after the start of the on- set, the eating difficulties gradually relieved, 45 to 60 days after removal of the particle chain, the upper gastrointestinal tract showed obstruction to lift. Patients were followed up for 7, 5, 3 and 2 months, respectively. One cases suffered from eating choke again at 7 months, and the other 3 cases were normal eating. Conclusion The esophageal 125I partical chain for advanced thoracic malignant tumors leading to eating difficulties caused by esophageal obstruction is elfective, minimally invasive, and it is possible to keep food unobstructed for a long time.
