萎胃康颗粒对幽门螺旋杆菌阳性慢性萎缩性胃炎患者血清GH、PGⅠ/PGⅡ的影响

摘　　要：目的:观察萎胃康颗粒对幽门螺旋杆菌(Hp)阳性慢性萎缩性胃炎血清生长激素调素(GH)、胃蛋白酶原(PG)的影响。方法:根据随机数字表法将Hp阳性的慢性萎缩性胃炎患者分为试验组和对照组,每组各53例。两组均給予常规四联疗法根除Hp(奥硝唑片0.5 g,克拉霉素0.5 g,雷贝拉唑片20 mg,枸橼酸铋钾胶囊0.6 g,2次/d,早晚各1次)连服用14 d后停用,对照组服用摩罗丹,试验组应用萎胃康颗粒,疗程3个月。观察两组患者的胃镜、病理组织学改变,及GH、胃蛋白酶原Ⅰ/胃蛋白酶原Ⅱ(PGⅠ/PGⅡ)的变化。结果:与对照组比较,试验组可明显减轻胃脘胀满、胃脘刺痛或隐痛、纳差食少、食后胃痛、神疲懒言、肢体倦怠、呃逆嗳气、烧心反酸、面色萎黄等症状(P<0.05);在改善胃黏膜萎缩程度方面试验组优于治疗组(P<0.05);治疗后试验组GH的含量(23.31±7.26)μg/L较对照组(19.89±6.72)μg/L增加明显(P<0.05);两组治疗后PGI/PGII在试验组为12.92±1.31、对照组8.03±0.98,试验组明显提高了PGI/PGII比值(P<0.05)。结论:萎胃康颗粒治疗慢性萎缩性胃炎的作用机制可能是通过提高PGI/PGII及GH水平,增强胃黏膜屏障,增加胃黏膜血流量,改善胃黏膜局部微循环,保护胃黏膜腺体,修复受损的黏膜。Objective： To observe the effect of Weiweikang Granules on the expressions of ghreli and PGⅠ/PGⅡ in chronic atrophic gastritis（ CAG）. Methods： A total of 106 CAG patients with Hp positive were assigned to the treatment group and the control group by random digit table,53 in each group. Patients in both groups took Ornidazole tablets（ 0. 5 g,bid,po）,Rabeprazole（ 20 mg,bid,po）,Bismuth Potassium Citrate Capsules（ 0. 6 g,bid,po）,clarithromycin（ 0. 5 g,bid,po）. Patients in treatment group took Weiweikang Granules,while patients in control group took Moluodan. The treatment course was 6 months for all. Clinical symptoms and signs,endoscopic and histopathological changes were observed before and after treatment in the two groups. The expressions of serum GH and PGⅠ/PGⅡin CAG patients were determined. The findings of gastroscopy and histology of gastric mucosa were recorded. Results： The symptoms of the two groups were significantly improved after treatment compared with those before treatment. Compared with the control group,improvments of bloating,belching,poor appetite,exhausted,anti acid hiccups belch,heartburn and sallow complexion（ P〈0. 05）. The improvement of the degree of gastric mucosal atrophy in experimental group was better than that in the treatment group（ P〈0. 05）. There was no diffenrence between the two groups in the expressions of GH and PGⅠ/PGⅡbefore treatment（ P〉0. 05）. After treatment of three months,the expressions of GH and PGⅠ/PGⅡ increased significantly（ P〈0. 05）. There was significant difference between two groups（ P〉0. 05）. Conclusion： Weiweikang Granules could improve chronic atrophic gastritis pathological points and significantly improve the pathological situation. Weiweikang Granules is an effective treatment for CAG and its mechanism is related to increasing the levels of GH and PGⅠ/PGⅡ.
