摘 要:目的:探讨Ki-67在胃癌中的表达及其与氟脲嘧啶类化疗药的相关性。方法:收集本院病理诊断明确的胃癌病人资料69例,且满足术后以氟脲嘧啶类药物为主的联合化学治疗,免疫组织化学方法检测胃癌病人Ki-67的表达情况及其与氟脲嘧啶类化疗药的相关性。结果:胃癌病人Ki-67阳性表达率随腺癌分化程度变化,分化程度越低表达越高(P<0.05),淋巴结转移病人组阳性表达率高于非淋巴结转移组(P<0.05),且与胃癌浸润深度正相关(P<0.05);同时Ki-67表达阳性病人临床有效率和疾病控制率明显高于表达阴性病人(P<0.05),Ki-67表达阳性率与氟脲嘧啶类药物敏感性相关(P<0.05)。结论:Ki-67的表达与胃癌组织病理类型、淋巴结转移、分化程度及氟脲嘧啶类药物化疗敏感性相关,Ki-67的表达可作为氟脲嘧啶类药物临床化疗选择方案的预测因子。Objective:To investigate the expression of Ki-67 in gastric cancer and its effect on the fluoropyrimidines drugs.Methods:Retrospective analysis was carried out in 69 gastric cancer patients,who were treated on fluoropyrimidine based combined chemotherapy after operation.the expression of Ki-67 was detected by immunohistochemical method,statistical analysis its expression in gastric carcinoma and its effect on fluorouracil chemotherapy drugs.Results:In gastric cancer patients,the Ki-67 positive expression rate were gradually increased,with the decrease of the degree of differentiation of gastric cancer(P〈0.05);Ki-67 positive expression rate in patients with lymph node metastasis was significantly higher than those in patients without lymph node metastasis(P〈0.05);Positive rate of Ki-67 expression in invasive cancer was higher than those in less invasive cancer.(P〈0.05).The clinical efficiency and disease control rate for gastric cancer patients with Ki-67 expression positive was significantly higher than that of patients with negative expression(P〈0.05).The positive rate of Ki-67 expression in the two groups(sensitive for fluoropyrimidines drugs vs insensitive groups)was statistically significant(P〈0.05).Conclusion:The expression of Ki-67 was correlated with the degree of differentiation,lymph node metastasis,infiltration depth and the sensitivity of fluoropyrimidines drugs.The expression of Ki-67 can be used as a predictive factor for the selection of fluoropyrimidines drugs.
