摘　　要：目的探索非小细胞肺癌(NSCLC)患者外周血循环肿瘤细胞(CTC)与临床病理和生存预后的关系。方法收集NSCLC 59例,术前采集外周血7.5 mL入Cellsave管,Cell Search系统检测患者CTC并计数,分析其与NSCLC临床病理因素和预后的关系。结果 NSCLC患者CTC阳性率为30.51%,CTC阳性与肿瘤远处转移(P=0.012)、TNM分期(P=0.002)、淋巴结转移(P=0.002)相关。CTC阳性与阴性组患者相比5年总生存率更低(P=0.005),术后5年累计复发率更高(P=0.022),单因素分析示TNM分期、淋巴结转移、远处转移及CTC阳性为影响NSCLC患者预后的因素;多因素分析示TNM分期、远处转移及CTC阳性影响预后。结论 NSCLC患者外周血循环肿瘤细胞阳性可作为评估预后的独立危险因素。Objective To investigate the value of circulating tumor cell （CTC） in non-small cell lung can- cer （NSCLC） and its relationship with clinicopathological features and prognosis. Methods This study totally in- cluded 59 patients with histologically confirmed diagnosis of NSCLC. CTCs were quantified in 7.5 mL peripheral blood samples collected prior to surgery using CellSearch system （Veridex）. Statistical analysis of the relationship between CTC positive rate and clinical data, pathological characteristics and prognosis were performed. Results The CTC positive rate was 30.51% and it was significantly correlated with distant metastasis （P = 0.012）, TNM stage （P = 0.002） and lymph node metastasis （P = 0.002）. The 5-year overall survival rate in the CTCs positive group was significantly lower （22.22% vs. 68.29%, P = 0.005） and the cumulative recurrence was significantly higher （55.55% vs. 29.27%, P = 0.022） compared with the CTCs negative group. The univariate analysis indicated that the prognostic influencing factors included TNM stage, lymph node metastasis, distant metastasis and CTCs re- sults and the multivariate analysis showed that TNM stage, distant metastasis and CTCs positive were significantly correlated with overall survival and cumulative recurrence （P 〈 0.05）. Conclusion CTC is an independent prog- nostic indicator for NSCLC.
