高强度聚焦超声消融治疗腹腔内型侵袭性纤维瘤7例护理

摘　　要：目的总结7例腹腔内型侵袭性纤维瘤患者行高强度聚焦超声(HIFU)消融治疗的护理资料,以规范此类患者护理要点。方法对7例行HIFU消融治疗的腹腔内型侵袭性纤维瘤患者于术前、术中及术后针对其个体情况进行具有专科特色的围手术期护理,并对观察记录的数据进行统计。结果治疗前主要症状:腹胀患者3例(43%)、压迫导致排尿困难患者2例(29%)、排便困难患者1例(14%)、腰背重度疼痛患者1例(14%)。治疗后不良反应:低热患者4例(57%)、治疗区轻-中度疼痛患者3例(43%)、治疗体位导致的前胸壁疼痛患者2例(29%)。通过围手术期针对性专科护理,7例患者治疗安全、有效,术后不良反应程度轻,无严重并发症发生,均痊愈出院。出院后对患者定期随访,不适症状均于治疗后3个月内完全缓解。结论具有专科特色的围手术期护理方案有望成为一种对HIFU消融治疗腹腔内型侵袭性纤维瘤有效的、可行的方法。Objective To establish a feasible nursing program for the patients undergoing HIFU for intraperitoneal invasive fibroma by summarizing the technical points of nursing in 7 cases. Method 7 patients with intraperitoneal invasive fibroma underwent perioperative specialized and individual nursing before, during and after HIFU treatment. Result The main symptoms before treatment were abdominal distension（3 cases, 43%）, dysuria（2 cases, 29%）, difficult defecation（1 case, 14%） and severe low back pain（1 case, 14%）. Adverse reactions after treatment included 4 cases（57%） of mild fever, 3 cases（43%） of mild to moderate pain in treatment sites, and 2 cases（29%） of anterior chest wall pain due to treatment posture. By specialized perioperative nursing, all 7 patients were treated by HIFU safely and effectively. Although there were mild postoperative adverse reactions, all patients were discharged after recovery without any serious complications. According to the follow-up results after the discharge, all symptoms were completely relieved within 3 months. Conclusion The specialized nursing program may be a feasible and effective option for patients undergoing HIFU for intraperitoneal invasive fibroma.
