减低剂量LMP方案联合α2b-干扰素治疗老年多发性骨髓瘤血清白蛋白浓度变化分析

摘　　要：目的观察骨髓瘤患者血清白蛋白浓度的变化,比较减低剂量LMP方案联合α2b-干扰素与标准MPT方案治疗老年多发性骨髓瘤的临床疗效。方法选取2009年1月—2013年12月在我院血液科就诊的老年多发性骨髓瘤患者96例,将其随机分为观察组与对照组,每组48例。观察组患者采取减低剂量LMP方案(雷利度胺、马法兰、泼尼松)加α2b-干扰素化疗;对照组采用标准MPT方案(马法兰,泼尼松,沙利度胺)化疗,在5个疗程后,评价二组的疗效,以及血清白蛋白浓度的变化。结果治疗后观察组血清白蛋白浓度(39.42±5.26)g/L高于对照组(36.81±6.14)g/L,差异有统计学意义(t=2.237,P=0.028)。观察组总有效率85.4%,对照组总有效率56.3%,二组比较差异有统计学意义(χ2=8.521,P=0.004)。结论减低剂量LMP方案联合α2b-干扰素较标准MPT方案治疗老年多发性骨髓瘤疗效更好。Objective To compare the effects of low dose LMP program （lenalidomide, melphalan, prednisone） in combination with regirrten and standard MPT program in the treatment of old patients with multiple myeloma by observing the change of 24 - hour urinary protein excretion.Methods Totally 96 patients were collected admitted to author＇s hospital from January 2009 to December 2013.The 96 patients were randomly divided into two groups,with each group 48 patients.The experimental group was treated with low dose LMP program and regimen, while the control group was treated with standard MPT program. Both groups were treated for 5 courses.Then we observed the therapeutic effects of the two groups,and compared the serum albumin of two groups.Results After treatment,the serum albumin of the experimental group was significantly higher than the control group （t = 2.237, P = 0.028） and the total efficiency of the experimental group was 85.4% with the control group 56.3% ,and the difference between the two groups was statistically significant （χ2 = 8.521,P = 0.004].Conclusion With low dose LMP program in combination with regimen can decrease serum albumin in the treatment of old patients with multiple myeloma and has a higher therapeutic effect on multiple myeloma than the standard MPT program.
