外生型肝癌的CT表现:2例报告并文献复习

摘　　要：目的探讨外生型肝癌的CT表现,提高对外生型肝癌的诊断水平及认识。方法回顾分析2例外生型肝癌病例的临床表现、实验室检查及多层螺旋CT表现,并结合文献进行分析。结果依据临床表现、实验室检查及常规B超、CT平扫难以准确诊断外生型肝癌,部分明显的外生型肝癌在CT像上有一定的特征性表现,术前CT提示诊断。结论 CT平扫显示肝脏周缘较大的等低密度、边缘尚清肿块,动态增强扫描可见肝内血管对其供血,大体符合"快进快出"表现,应考虑为外生型肝癌。Objective To explore CT features of pedunculated hepatocellular carcinoma（PHCC） and improve the diagnostic level of PHCC. Methods The clinical manifestations, laboratory tests and multi-slice spiral CT features of 2 cases with PHCC were retrospectively analyzed with literatures analysis. Results It was difficult to accurately diagnose PHCC based on clinical manifestations, laboratory test, Conventional ultrasound and CT plain scan. Some obvious PHCC have special features in CT images. Preoperative CT can prompt diagnosis. Conclusion CT scan showed a low density large liver periphery with clear edge mass.Dynamic enhanced CT scan showed blood supply from the intrahepatic vessels. It generally conform to the ＂fast forward and fast out＂ performance. It should be diagnosed as PHCC.
