溃洁灌肠液治疗Ⅱ度放射性直肠炎的临床疗效

摘　　要：目的探讨溃洁灌肠液治疗Ⅱ度放射性直肠炎的临床疗效。方法选取南京中医药大学第三附属医院肛肠科及肿瘤科2011年1月—2017年3月收治的Ⅱ度放射性直肠炎患者195例,随机分为A组、B组和C组,每组65例。A组患者给予单纯西药治疗,B组患者给予康复新+地塞米松灌肠治疗,C组患者给予溃洁灌肠液保留灌肠。比较3组患者的临床疗效,治疗前后症状(腹泻、腹痛、血便)积分及血液流变学指标[中性粒细胞计数(NE)、红细胞沉降率、C反应蛋白(CRP)、丙氨酸氨基转移酶(ALT)],并观察患者不良反应发生情况。结果 C组患者治疗总有效率高于A组和B组,B组患者治疗总有效率高于A组(P<0.05)。治疗前,3组患者腹泻、腹痛、血便积分比较,差异无统计学意义(P>0.05);治疗后,B组和C组患者腹泻、腹痛、血便积分低于A组,C组患者腹泻、腹痛、血便积分低于B组(P<0.05)。治疗前,3组患者NE、红细胞沉降率、CRP、ALT比较,差异无统计学意义(P>0.05)。治疗后,B组和C组患者NE、红细胞沉降率、CRP低于A组(P<0.05);C组患者NE、红细胞沉降率、CRP低于B组(P<0.05),3组患者ALT比较,差异无统计学意义(P>0.05)。3组患者均未出现严重不良反应。结论采用溃洁灌肠液治疗Ⅱ度放射性直肠炎的临床疗效确切,其可更有效改善患者的临床症状,安全有效。Objective To observe the curative effect of kuijieguanchangye in the treatment of degree Ⅱ radiation proctitis. Methods A total of 195 cases of degree Ⅱ radioactive proctitis patients were selected from January 2011 to March2017 in Third Affiliated Hospital of Nanjing University of Traditional Chinese Medicine,which were randomly divided into group A,group B and group C,65 cases in each group. Group A was given simple western medicine,group B was given new rehabilitation ＋ dexamethasone enema,group C was given kuijieguanchangye. The clinical effect,clinical symptoms（ diarrhea,stomachache,bloody stool） scores and hemodynamic indexes（ NE,erythrocyte sedimentation rate,CRP,ALT） before and after treatment were compared among three groups,and the incidence of adverse reactions were observed. Results The total effective rate of group C was higher than group A,group B,the total effective rate of group B was higher than group A（ P 〈 0. 05）.Before treatment,no statistically significant differences of diarrhea,stomachache,bloody stool scores was found among the three groups（ P 〈 0. 05）; after treatment,the diarrhea,stomachache,bloody stool scores of group B and group C were lower than group A,the diarrhea,stomachache,bloody stool scores of group C were lower than group B（ P 〈 0. 05）. Before treatment,no statistically significant differences of NE,erythrocyte sedimentation rate,CRP or ALT was found among the three groups（ P 0. 05）; after treatment,the NE,erythrocyte sedimentation rate,CRP of group B and group C were lower than group A,the NE,erythrocyte sedimentation rate,CRP of group C were lower than group B（ P 〈 0. 05）. No statistically significant difference of ALT was found among the three groups（ P 〉0. 05）. No obvious complications happened. Conclusion Retention enema juice retention enema has a definite effect in treating degree Ⅱ radiation proctitis,and it is safe and effective.
