养阴清肺汤对乳腺癌术后放射性肺炎患者氧化应激反应及免疫功能影响研究

摘　　要：目的 :探讨养阴清肺汤对乳腺癌术后放射性肺炎患者氧化应激反应及免疫功能的影响。方法 :选取医院2015年2月—2017年1月收治的乳腺癌术后放射性肺炎患者158例,按随机数字表法分组,对照组79例予以常规对症治疗,研究组79例在对照组基础上予以养阴清肺汤治疗,观察并记录两组间血清氧化应激反应相关指标、血液T淋巴细胞亚群、血清细胞因子、血清纤维化指标水平,同时比较临床疗效。结果 :对照组总有效率(75.96%)低于研究组总有效率(88.61%)(P<0.05)。与对照组比较,研究组治疗后血清丙二醛(MDA)水平较低,超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-PX)水平较高,治疗后血CD_3^+、CD_4^+T淋巴细胞,自然杀伤(NK)细胞,CD_4^+/CD_8^+水平较高,血CD_8^+T淋巴细胞水平较低,治疗后血清转化生长因子-β_1(TGF-β_1)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、C反应蛋白(CRP)水平较低,治疗后血清Ⅰ型前胶原(PⅠCP)、Ⅲ型前胶原(PⅢNP)、层黏蛋白(LN)水平较低,具有统计学意义(P<0.05)。结论 :养阴清肺汤治疗放射性肺炎的疗效较好,能够有效提高患者抗氧化能力,减轻氧化应激反应,改善细胞免疫功能,抑制炎症反应,防治肺纤维化的发生,有助于改善患者预后。Objective ：To investigate the effects of Yangyin Qingfei Decoction on oxidative stress and immune function in patients with radiation pneumonitis after breast cancer.Methods ：158 patients with advanced radiation pneumonitis from February 2015 to January 2017 in our hospital were selected and divided into 2 groups,79 cases in the control group treated with conventional symptomatic treatment,79 cases in the experiment group treated with Yangyin Qingfei Decoction on the basis of the control group,the levels of serum oxidative stress related indexes,blood T lymphocyte subsets,serum cytokines and serum fibrosis markers,and the clinical effect were compared after the treatment.Results ：The effective rate in the control group（75.96%）was lower than that in the experiment group（88.61%）,with significant difference（P〈0.05）;compared with the control group,serum levels of malondialdehyde（MDA）was lower,levels of superoxide dismutase（SOD）and glutathione peroxidase（GSH-PX）were higher after treatment in the experiment group,blood levels of CD3^＋,CD4^＋ T lymphocytes,natural killer（NK）cells and CD4^＋/CD8^＋ were higher,blood levels of CD8^＋ T lymphocytes was lower after treatment,serum transforming growth factorβ1（TGF-β1）,tumor necrosis factorα（TNF-α）,interleukin-6（IL-6）,C reactive protein（CRP）levels were lower after treatment,serum procollagen type I（PICP）,type III procollagen（PⅢNP）,laminin（LN）levels were lower after treatment,with significant difference（P〈0.05）.Conclusion ：The clinical effect of Yangyin Qingfei Decoction in the treatment of radiation pneumonitis is preferably,it can significantly improve the antioxidant capacity of patients,reduce oxidative stress response,improve cellular immune function,inhibit inflammation and prevent the occurrence of pulmonary fibrosis,help to improve the prognosis of patients.
