幽门螺杆菌感染与大肠肿瘤的发生及临床病理特点的关系

摘　　要：目的探讨幽门螺杆菌感染与大肠肿瘤的发生及其临床病理特点的关系,为幽门螺杆菌在大肠肿瘤发病中的作用提供依据。方法选择2014年1月-2016年12月医院收治的大肠癌患者70例作为大肠癌组,大肠腺瘤患者70例作为大肠腺瘤组,结直肠镜及组织病理学结果正常者70名作为对照组;统计分析三组临床病理特点资料。结果大肠癌组和大肠腺瘤组幽门螺杆菌感染率分别为68.6%、71.4%,高于对照组的51.4%(P<0.05);大肠癌组患者幽门螺杆菌感染与患者的性别、年龄、肿瘤大小、病理分级、病理类型、淋巴结转移无关;大肠腺瘤组患者幽门螺杆菌感染与患者性别、年龄、肿瘤大小、肿瘤数目、肿瘤蒂部情况、肿瘤病理分型无关。结论幽门螺杆菌感染与大肠癌及大肠腺瘤的发生有关,但与大肠癌及大肠腺瘤的临床病理特点无关。OBJECTIVE To investigate the relationship between Helicobacter pylori infections and colorectal neoplasms and its clinicopathological features,so as to provide evidence for the role of H.pylori in the pathogenesis of colorectal cancer.METHODS A total of 70 patients with colorectal cancer from Jan.2014 to Dec.2016 were selected as colorectal cancer group,70 patients with colorectal adenoma were selected as colorectal adenoma group,and 70 patients with normal colorectal neoplasms and histopathological results were selected as control group.The clinical and pathological characteristics of the three groups were statistically analyzed.RESULTS The infection rates of H.pylori in colorectal cancer group and colorectal adenoma group were 68.6% and 71.4%,which were higher than 51.4%in control group（P〈0.05）.The infection of H.pylori in patients with colorectal cancer was not associated with genders,age,tumor sizes,pathological grades,pathological type and lymph node metastasis.The infection of H.pylori in colorectal adenoma group was not correlated with genders,age,tumor sizes,tumor number,tumor pedicle and tumor pathological types.CONCLUSION H.pylori infection is related to colorectal cancer and colorectal adenoma,but has nothing to do with the clinical and pathological features of the colorectal cancer and colorectal adenoma.
