鼻咽癌患者放射性猛性龋咬合重建的修复模式探讨

摘　　要：目的探讨放射性猛性龋咬合重建的修复模式,为要求尽快恢复咀嚼功能进行咬合重建的鼻咽癌患者提供临床理论依据。方法选择14例鼻咽癌放疗术后的猛性龋损致咀嚼功能丧失的患者。在同一位患者口内用三种修复方式分三个阶段依次进行:修复1,修复2,修复3。每组修复模式均试戴2个月适应后采用分光光度法测定咀嚼效能,同时采用满意度问卷调查获取患者的主观评价。结果经过修复后患者咀嚼满意度显著提高,修复前满意度为21.43%,修复1和修复2的满意度均为85.71%,修复3的满意度为92.86%,修复前后比较,存在统计学意义(P<0.05);三组数据比较无统计学差异(P>0.05)。结论三种修复模式均可作为终末修复,都能恢复比较好的咀嚼功能,让患者能根据自己的经济条件选择修复方案、有计划地分期治疗,尽快恢复咬合功能。m Objective To investigate the repair model of occlusal reconstruction of radiation induced caries, and to provide theoretical basis for the reconstruction of masticatory function as soon as possible for the patients with nasopharyngeal carcinoma.Methods 14 cases of masticatory loss due to rampant caries after radiotherapy for nasopharyngeal carcinoma were selected. In the same patient＇s mouth, three repair methods were performed in three stages： repair 1, repair 2, repair 3. Each group was tested for 2 months after adjustment, and the masticatory efficiency was measured by spectrophotometry. The subjective evaluation of the patients was conducted by satisfaction questionnaire.Results After repair of chewing satisfaction improved significantly, before restoration satisfaction was 21.43%, 1 and 2 of the repair repair repair satisfaction was 85.71%, 3 satisfaction was 92.86%, before and after the restoration of comparison, there were statistical significance （P〈0.05）; no significant differences between the three groups of data （P〉 0.05）.Conclusion Three kinds of repair models can be used as terminal restoration, and can restore good chewing function, so that patients can choose repair plan according to their own economic conditions, plan treatment in stages, and restore occlusal function as soon as possible.
