USCOM监测仪对心力衰竭患儿血流动力学的监测及其临床意义

摘　　要：目的探讨便携式超声心输出量监测仪(ultrasonic cardiac output monitor,USCOM)在心衰患儿血流动力学变化中的应用及临床价值。方法对我院收治的28例心力衰竭患儿应用USCOM监测仪监测相关血流动力学数据,包括心率(HR)、每搏输出量(SV)、外周血管阻力(SVR)、心肌收缩力指数(INO)等,判断其前负荷、后负荷以及心肌收缩力情况,以指导临床治疗用药。结果通过积极病因判断以及药物治疗后,28例心衰患儿中25例恢复良好,2例放弃治疗,1例死亡,总有效率为89.3%,整体预后良好。结论 USCOM监测仪对患儿心衰原因的判断、液体控制及正性肌力药、利尿剂、血管扩张药的使用具有一定指导作用。Objective To explore the application and clinical value of the portable ultrasonic cardiac output monitor(USCOM)for monitoring hemodynamic changes in the children with heart failure. Methods 28 cases of children with heart failure in our hospital were used the USCOM to monitoring related the hemodynamics data,including the heart rate(HR),stroke volume(SV),peripheral vascular resistance(SVR),and myocardial contraction index(INO),etc,judge the condition of preload,afterload and myocardial contractility to guide the clinical medication. Results 25 of 28 cases in children with heart failure were recovered well,other 2 cases were given up treatment,and 1 case was death.Total effective rate was 89.3%. Conclusion USCOM has a certain guiding role to determine the cause of children with heart failure,fluid control,and medicine of positive myodynamia,diuretics,and use of vasodilators.
