摘  要:目的:探讨三叉神经痛应用微血管减压术(MVD)与感觉根部分切断术(PSR)联合治疗的临床疗效。方法:将90例原发性三叉神经痛患者随机分为对照组(45例)与研究组(45例),对照组患者手术仅采用MVD治疗,研究组手术治疗联用MVD和PSR。比较术后1周患者的临床疗效,随访24个月并观察随访期内的患者疼痛消除和并发症发生情况。结果:术后1周研究组患者治疗的总有效率为97.78%,显著高于对照组的86.67%(P<0.05);研究组疼痛消除率为93.33%,明显大于对照组的75.56%(P<0.05);对照组并发症发生率为13.33%,与研究组的11.11%相比无统计学差异(P>0.05)。结论:MVD与PSR联合治疗原发性三叉神经痛的安全性高,能有效缓解疼痛,提高远期疗效。Objective:To investigate the clinical effect of combined treatment of trigeminal neuralgia with microvascular decompression(MVD)and partial root sensory excision(PSR).Methods:Ninety patients with primary trigeminal neuralgia were randomly divided into control group(45 case)and study group(45 case).Patients in the control group were treated with MVD only.The study group was treated with MVD and PSR.The clinical efficacy of the patients was compared at 1 week after operation.The patients were followed up for 24 months and the pain relief and complication occurred during the follow-up period.Results:The total effective rate of the study group was 97.78%at 1 week after operation,which was significantly higher than that of the control group 86.67%(P<0.05).The pain relief rate was 93.33%in the study group,significantly higher than that in the control group 75.56%(P<0.05).The incidence of complications in the control group was 13.33%,which was not significantly different from that in the study group 11.11%(P>0.05).Conclusion:The combined treatment of MVD and PSR is safe and safe for primary trigeminal neuralgia,which can effectively relieve the pain and improve the long-term curative effect.
