摘 要:目的探讨光动力治疗(photodynamic therapy,PDT)对复发脑胶质瘤出现再复发的影响,并分析其与MMP2表达的关系。方法回顾脑胶质瘤首次复发病例33例,根据术中是否采用PDT分为PDT组(n=17)和非PDT组(n=16)。收集与硬脑膜黏连的肿瘤组织,通过Western blotting法、免疫组织化学法分别检测MMP2蛋白表达和阳性指数,并分析再复发时间与病人临床特点的相关性。结果与非PDT组相比,PDT组MMP2蛋白表达和阳性指数显著降低(P<0.05),1年内再复发例数明显减少(P<0.05)。脑胶质瘤病理分级越低,1年内再复发可能性越小(r=0.902,P<0.05)。MMP2表达越低,1年内再复发可能性也越小(r=0.587,P<0.05)。结论 PDT可降低复发脑胶质瘤MMP2的表达,延长病人再复发的时间。Objective To investigate the effect of photodynamic therapy(PDT) on recurrence of recurrent glioma patients and analyze the relationship with MMP2 expression. Methods The clinical data of 33 glioma patients after first relapse were analyzed retrospectively. According to whether the use of PDT during surgery, the patients were divided into 2 groups: PDT group(n=17) and non PDT group(N-PDT, n=16). The tumor tissues adhered with dura mater were collected to detect the expression of MMP2 protein and positive index by Western blotting and immunohistochemistry method, respectively. And the correlation between recurrence time and clinical characteristics was analyzed. Results Compared with N-PDT group, MMP2 protein expression and the positive index in the PDT group were decreased significantly(P〈0.05), and the one-year recurrence decreased significantly(P〈0.05). The lower the pathological grading of glioma, the lower the possibility of re-recurrence within 1 year(r=0.902, P〈0.05). And the lower the expression of MMP2, the lower the possibility of re-recurrence within 1 year(r=0.587, P〈0.05). Conclusion PDT can decrease the MMP2 expression of recurrent glioma and prolong the recurrence time.
