摘　　要：目的探讨侵及颅底腺样囊性癌的临床特征。方法回顾性分析18例侵及颅底腺样囊性癌临床资料,初治病例7例,复发病例11例。通过电话、门诊复查等方式随访。采用Kaplan-Meier法绘制生存期、无进展生存期曲线。结果在18例病人中,死亡9例,出现局部复发9例;新发远处转移5例。中位生存期为57.0个月,中位无进展生存期为23.3个月。在初治病例中,出现局部复发时间和远处转移时间,术后辅助放疗病人较未放疗病人长。结论侵及颅底腺样囊性癌发病率低,治疗应选择手术联合辅助放疗。但复发病例常因既往治疗史复杂导致治疗困难。局部复发与远处转移率高,病人预后差。Objective To explore the clinical features of adenoid cystic carcinoma with the skull base invasion. Methods The clinical data of 18 patients with adenoid cystic carcinoma with skull base invasion were analyzed retrospectively. All the patients were followed up by telephone and outpatient. Overall survival and progression-free survival was calculated by the Kaplan-Meier curve. Results Death occurred in 9 patients, local recurrence in 9 and distant metastasis in 5 among 18 patients during follow-up. Median time of overall survival and progression-free survival was 57.0 months and 23.3 months respectively. In the newly diagnosed cases, patients with postoperative adjuvant radiotherapy owned significantly longer local recurrence and distant metastasis time than those without radiotherapy. Conclusions Adenoid cystic carcinoma with the skull base invasion is rare. The best therapeutic regimen is radical surgery followed by radiation therapy. However, it is difficult to make choice of treatment due to the complicated treatment history of recurrent cases. Patients with adenoid cystic carcinoma with the skull base invasion has poor prognosis with relatively high local recurrence and distance metastasis rate.
