摘　　要：目的 研究中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)、红细胞分布宽度(RDW)和癌胚抗原(CEA)对结直肠癌TNM分期的预判价值。方法 回顾性分析92例结直肠癌患者临床资料。应用Kruskal-Wallis秩和检验分析NLR、PLR、RDW和CEA值与结直肠癌TNM分期的关系,并行Pearson相关性分析。结果 结直肠癌TNM分期Ⅰ期7例,Ⅱ期33例,Ⅲ期34例,Ⅳ期18例。TNM分期Ⅰ~Ⅳ期NLR中位数值分别为2.53,2.37,2.69和4.57(P=0.010);PLK中位数值分别为114.91,157.41,153.44和183.22(P=0.106);RDW中位数值分别为41.80,43.30,42.40和45.85(P=0.064);CEA中位数值分别为3.60,3.20,7.75和15.15(P=0.007)。Spearmann相关性分析结果显示相关系数NLR>CEA>PLR>RDW,NLR、CEA、PLR值与结直肠癌TNM分期明显正相关(P<0.05)。结论 NLR、PLR和CEA值在预测结直肠癌患者TNM分期上具有一定的临床意义,可为临床提供参考。Objective To investigate the predicting value of neutrophil-lymphocyte ratio （NLR）, platelet-lymphocyte ratio （PLR）, red cell volume distribution width （RDW） and carcino-embryonie antigen （CEA） in the colorectal tumor TNM Staging. Method The clinical data of 92 hospitalized patients with colorectal tumor from digestive and general surgery department between January 2015 and July 2017 were retrospectively analyzed. Kruskal-Wallis test and Pearson correlation analysis were used to analysis the relationship between the values of NLR, PLR, RDW, CEA and coloreetal tumor TNM staging. Result 7 cases stage I , 33 cases stage Ⅱ , 34 cases stage Ⅲ ,and 18 cases stage IV were entered. Results of TNM staging from stage Ⅰ to IV were as follows： the median values of NLR, 2.53, 2.37, 2.69 and 4.57（P=0.010） ; the median values of PLR, 14.91, 157.41, 153.44 and 183.22 （P= 0. 106） ; the median values of RDW, 41.80, 43.30, 42.40 and 45.85 fl （ P = 0. 064） ; the median values of CEA, 3.60, 3.20, 7.75 and 15.15 ng/ml （ P = 0.007 ）, respectively. Spearmann correlation analysis showed significantly positive correlation between NLR, CEA, PLR and eoloreetal tumor TNM staging P〈0.05 ）, and correlation coeffi- cient NLR〉CEA〉PLR〉RDW. Conclusion NLR, PLR and CEA might has certain clinical significance for predicting the TNM staging of eoloreetal tumor , which will play a reference role in clinical application.
