食管内覆膜支架置入术治疗食管癌梗阻及相关并发症的临床观察

摘　　要：目的观察分析覆膜支架治疗食管癌梗阻及相关并发症的临床效果。方法本组54例,其中食管癌梗阻23例,食管气管瘘17例,食管胸膜腔瘘6例,食管纵隔瘘8例。对于单纯梗阻者采用覆膜支架食管内置入术,其并发症采用覆膜支架食管内置入术,同时行胸腔闭式引流术,纵隔内置管灌洗引流术,7天后复查评价疗效。结果本组食管癌梗阻并发瘘54例,其中显效33例(61.11%),有效19例(35.18%),无效2例(3.70%),总有效率为96.30%(52/54)。结论覆膜支架治疗食管癌梗阻及其并发症(食管气管瘘、食管胸膜腔瘘、食管纵隔瘘等),疗效确切,值得借鉴。Objective To observe and analyze the clinical effect of covered stent in the treatment of esophageal carcino-ma obstruction and related complications. Methods There were 54 cases of esophageal cancer obstruction, 23 cases of esopha-geal fistula, 6 cases of esophageal pleural fistula, 17 cases of esophageal and mediastinal fistula, 8 cases of esophageal fistula. For the patients with simple obstruction, the covered stent and esophagus were inserted into the esophagus〉 and the complica-tions were treated with covered stent and esophagus. At the same time, closed thoracic drainage？ lavage and drainage of medi-astinum with internal tube were performed. The curative effect was evaluated after 7 days. Results There were 54 cases of e- sophageal cancer obstruction complicated with fistula, among which 33 cases were markedly effective （61. 11%） , effective in 19 cases （35. 18%） , ineffective in 2 cases （3. 70%） , and the total effective rate was 96. 30% （52/54）. Conclusion Covered stent for the treatment of esophageal cancer obstruction and its complications （esophago tracheal fistula, esophageal pleural fistula, esophageal mediastinal fistula, etc. ） was accurate and worthy of reference.
