术前短程辅助化疗联合根治性手术治疗结肠癌的临床效果及对患者CDX2、ING4表达的影响

摘　　要：目的探究术前短程辅助化疗联合根治性手术治疗结肠癌的临床效果及对CDX2、ING4基因表达的影响。方法选取结肠癌患者90例,随机分为观察组和对照组各45例。对照组采用单纯根治性手术治疗,观察组在手术前应用奥沙利铂、5氟尿嘧啶、亚叶酸钙进行短程冲击化疗。统计2组短期疗效、长期疗效及不良反应发生情况,并对比2组CDX2、ING4表达情况。结果观察组肿瘤切除率和直肠癌保肛率均显著高于对照组(P<0.05)。2组入院时CDX2、ING4基因阳性率差异均无统计学意义(P>0.05);观察组手术时及术后3个月CDX2、ING4基因阳性率显著高于对照组(P<0.05);观察组手术后1年和2年的肿瘤复发率显著低于对照组(P<0.05),但第3年2组的肿瘤复发率差异无统计学意义(P>0.05)。结论在根治性手术前联合短程辅助化疗可有效提高临床效果,调节CDX2、ING4基因表达,并具有一定的安全性。Objective To explore the clinical effect of preoperative short-term adjuvant chemotherapy combined with radical surgery in the treatment of colorectal cancer and its effect on caudal type homeobox transcriptionfactor 2(CDX2)and inhibitor of growthfamily member 4(ING4)expression.Methods A total of 90 patients with colorectal cancer were selected.They were divided into observation group and control group.The patients in the control group were treated with simple radical surgery,and the observation group was treated with oxaliplatin,pentafluorouracil and leucovorin for short-term impact chemotherapy before operation.The shortterm efficacy,long-term efficacy and adverse events were observed in both groups.And the expression of CDX2 and ING4 in the two groups were compared.Results The tumor resection rate and rectal cancer in the observation group were significantly higher than those in the control group(P<0.05).There was no significant difference in the positive rate of CDX2 and ING4 gene between two groups before treatment(P>0.05).The positive rate between two indicators in the observation group was significantly higher than those at the time of operation and 3 months after operation(P<0.05).The positive rate of CDX2 and ING4 expression in the observation group was significantly higher than those in the control group(P<0.05).There was no significant change in the positive rate between two groups(P>0.05).The recurrence rate was significantly lower in the observation group than in the control group at 1 year and 2 years after operation(P<0.05),but there was no significant difference in tumor recurrence rate between two groups(P>0.05).Conclusion The radical surgery combined with short-term adjuvant chemotherapy can effectively improve the clinical effect,regulate CDX2 and ING4 gene expression,which has a certain safety.
