摘 要:目的探讨X线摄影和超声结合与增强磁共振两种方法诊断乳腺癌的价值。方法收集术前完成乳腺X线摄影、乳腺超声和乳腺增强磁共振检查并病理证实乳腺癌患者840例。利用美国放射学院的BI-RADS分级,对比分析X线乳腺摄影与超声结合、增强磁共振检查的准确率、错误率及敏感度。统计方法采用卡方检验。结果 X线乳腺摄影与超声联合诊断的准确率、Ⅱ,Ⅲ类错误率、Ⅳ,Ⅴ类敏感度分别为80.00%、1.42%、15.71%、42.85%、37.14%。增强磁共振的准确率、Ⅱ,Ⅲ类错误率、Ⅳ,Ⅴ类敏感度分别为:87.85%、1.42%、10.71%、50.71%、37.14%。经卡方检验,P=0.000<0.05,差异有统计学意义,可以认为增强磁共振的准确度、Ⅳ类敏感度均高于X线乳腺摄影与超声联合诊断。Ⅴ类敏感度两种方法相同。结论增强磁共振对乳腺癌的诊断优于乳腺X线摄影和超声联合诊断。Objective To compare the diagnostic efficacy for breast cancer by the combined method of ultrasound and mammography with enhanced MRI examination. Methods 840 consecutive patients with pathologically confirmed breast cancer were enrolled. All patients had breast ultrasound,mammography and enhanced MR examinations. The accuracy,error rate,sensitivity of each modality were compared. Results The accuracy,Ⅱ,Ⅲ error rate and Ⅳ,Ⅴ sensitivity of the combined method of ultrasound and mammography were 80. 00%,1. 42%,15. 71%,42. 85%,37. 14. Respectively,and those of enhanced MR examination,were 87. 85%,1. 42%,10. 71%,50. 71%,37. 14%. The accuracy,and Ⅳ sensitivity of enhanced MR examination were all higher than those of the combined method( all P〈0. 05). Conclusion Enhanced MR examination is superior to the combined method of ultrasound and mammography in accuracy,sensitivity in breast cancer examination.
