完全腹腔镜下不切断空肠Roux-en-Y吻合在远端胃癌根治术中的实用性研究

摘　　要：目的探讨完全腹腔镜下uncut Roux-en-Y吻合术在远端胃癌根治手术中应用的安全性及可行性。方法回顾性分析2016年2月~2017年5月潍坊市人民医院肿瘤外科实施的全腹腔镜远端胃癌D2根治术(Uncut Roux-en-Y吻合)35例临床资料。结果 35例患者手术全部在完全腹腔镜下进行,无中转开放病例,手术平均时间212.08±24.66min,淋巴结清扫总数31.5±9.2枚,消化道重建时间55.72±12.20min,术中平均失血为60.72±26.07ml,术后平均排气时间3.91±0.84d,进食半流质时间4.89±0.61d,手术后平均住院7.58±0.89d。2例患者出现术后并发症,予以保守治疗后均好转出院,无其他相关术后并发症发生。结论完全腹腔镜下uncut Roux-en-Y吻合术式安全可行,具有视野清晰、原位操作、切口小、恢复快等优点。Objective To investigate the safety and feasibility of uncut Roux-en-Y anastomosis in complete laparoscopic surgery for distal gastric cancer. Methods Retrospective analysis of February 2016 to May 2017 weifang city people＇s hospital of tumor surgery on the implementation of total laparoscopic distal gastric cancer D2 radical surgery（ uncut Roux-en-Y anastomosis） 35 cases of clinical data. Results The average time of operation was 212. 08 ± 24.66 min,the total number of lymph node dissection was 31. 5 ± 9. 2,the reconstruction time of digestive tract was 55. 72 ±12. 20 min,and the average blood loss was 60. 72 ± 26. 07 ml in the operation. The average postoperative ventilation time was 3. 91 ± 0. 84 days,the semi-fluid time was 4. 89 ± 0. 61 days,and the average hospital stay was 7. 58 ± 0. 89 days after operation. Two patients with postoperative complications,to be conservative treatment were discharged after discharge.No other related postoperative complications occurred. Conclusion Uncut Roux-en-Y is completely safe and feasible,and has the advantages of clear field of view,in situ operation,small incision and quick recovery.
