31例原发性输尿管癌诊治的临床观察

摘　　要：目的通过观察原发性输尿管癌诊断、手术治疗、药物治疗及术后随诊、随访情况,了解手术对原发性输尿管癌的疗效及术后5年患者生存情况。方法观察经过诊断及手术治疗和药物治疗的31例原发性输尿管癌患者的临床资料,通过随诊、随访,了解患者术后生存情况。结果 31例患者均完成输尿管癌根治术,术后恢复良好。术后病理检查尿路移行细胞癌30例,鳞状细胞癌1例。31例患者手术后1年内生存28例,1年生存率为90.3%;2年内生存25例,2年生存率为80.6%;3年内生存21例,3年生存率为67.7%;手术5年后生存17例,5年生存率为54.8%。结论原发性输尿管癌预后差,5年生存率低。提高原发性输尿管癌的5年生存率,早期诊断非常重要,在早期诊断的基础上进行综合治疗,是目前的有效手段。Objective To understand the efficacy of operation on primary ureteral carcinoma and survival of postoperative 5 year by observing the diagnosis, surgical treatment, drug treatment, postoperative follow-up and follow-up of primary ureteral carcinoma. Methods The clinical data of 31 cases of primary ureteral carcinoma after diagnosis, operation and drug treatment were observed, and to understand the survival condition of patients by follow-up visiting. Results All the 31 patients completed radical resection of ureteral carcinoma and recovered well after operation. Pathological examination showed 30 cases of transitional cell carcinoma of the urinary tract and 1 case of squamous cell carcinoma. 28 patients survived within 1 year after operation in 31 cases, with 1 year survival rate as 90.3%, 25 patients survived within 2 years, with 2-year survival rate as 80.6%, 21 patients survived within 3 years, with 3-year survival rate as 67.7%. 17 patients survived within 5 years, with 5-year survival rate as 54.8%. Conclusion The prognosis of primary ureteral carcinoma is poor, and the 5 year survival rate is low. Early diagnosis is very important to improve the 5 year survival rate of primary ureteral carcinoma. Comprehensive treatment on the basis of early diagnosis is an effective method.
