摘 要:目的研究胆囊癌患者癌组织中趋化因子CXCL12以及其受体CXCR4/CXCR7的表达与意义。方法选取2012年4月至2016年6月在我院行手术切除治疗的67例证实为胆囊癌的标本作为观察组,同时另选取67例正常胆囊组织标本作为对照组。采用免疫组织化学法(SP)检测癌组织中趋化因子CXCL12及其受体CXCR4、CXCR7表达水平及意义。结果观察组患者CXCL12、CXCR4与CXCR7的表达率明显高于对照组,差异有统计学意义(P<0.05);趋化因子CXCL12及其受体CXCR4、CXCR7在性别、年龄中比较差异无统计学意义(P>0.05),在中低分化中阳性表达率明显高于在高分化的阳性表达率,差异有统计学意义(P<0.05),在有淋巴结转移中的阳性表达率明显高于无淋巴结转移中的阳性表达率,在TNM分期中Ⅲ+Ⅳ期的阳性表达率明显高于Ⅰ+Ⅱ期的阳性表达率(P<0.05);经Pearson相关性分析显示,CXCL12与CXCR4蛋白在胆囊癌中成正相关(r=0.424,P<0.05),CXCL12与CXCR7蛋白在胆囊癌中成正相关(r=0.624,P<0.05),CXCL4与CXCR7蛋白在胆囊癌中无相关(r=0.164,P>0.05)。结论趋化因子CXCL12及其受体CXCR4、CXCR7在胆囊癌组织中高度表达,而且与TNM分期、淋巴结转移、组织学分级有着密切的相关性,对胆囊癌诊断与预后具有一定的临床价值。Objective To investigate the expression and significance of chemokine CXCL12 and its receptor CXCR4/CX- CR7 intissues of gallbladder carcinoma. Methods 67 cases of surgical resection proved to be gallbladder carcinoma in our hospital from April 2012 to June 2016were selected as the observation group, and 67 normal gallbladder tissue samples were selected as the control group. Immunohistochemical method(SP) was used to detect the expression of chemokine CXCL12 and its receptor CXCR4, CXCR7 in cancer tissues and its significance. Results The expression rates of CXCL12, CXCR4 and CXCR7 in the observation group were significantly higher than those in the control group (P 〈 0. 05) ;The chemokine CXCL12 and its receptor CXCR4, CX- CR7 were not significantly different in gender and age (P 〉 0. 05 ), the positive expression rate in the middle and low differentiation group was significantly higher than that in the high differentiation rate( P 〈 0.05 ) ,the positive expression rate in lymph node me- tastasis was significantly higher than that without lymph node metastasis ,in TNM staging, the positive expression rate of stage III + IV was significantly higher than that in stage I + II (P 〈 0. 05 ) ;Pearson correlation analysis showed that CXCL12 and CXCR4 pro- tein were positively correlated in gallbladder carcinoma( r = 0. 424 ,P 〈 0.05 ), CXCL12 and CXCR7 protein were positively corre- lated in gallbladder carcinoma( r = 0. 624 ,P 〈 0.05 ), no correlation between CXCIA and CXCR7 protein in gallbladder carcinoma ( r = 0. 164, P 〉 0. 05 ). Conclusion Chemokine CXCL12 and its receptor CXCR4, CXCR7 are highly expressed in tissues of gall- bladder carcinoma,and they are closely related to TNM stage, lymph node metastasis and histological grade, which have certainclinical value for the diagnosis and prognosis of gallbladder carcinoma.
