摘　　要：目的探讨配对血浆滤过吸附(CPFA)技术对ICU收治重症急性胰腺炎(SAP)病人内环境稳态和肝肾功能影响。方法将52例SAP病人随机分为观察组和对照组(n=26),对照组采用ICU常规治疗方案,观察组在常规治疗方案基础上采用配对血浆滤过吸附技术,观察两组病人ICU住院期间肝肾功能变化及急性生理与慢性健康状况评分II(APACHE II)和内环境稳态的变化。结果两组病人ICU治疗3d、7d后分别检测血白细胞计数、肝功能(谷丙转氨酶、谷草转氨酶和r-谷氨酰转肽酶、白蛋白、总胆红素水平)、肾功能(尿素氮、血肌酐)和APACHE II评分,观察组的指标变化均优于对照组,差异具有显著性意义(P<0.05)。结论配对血浆滤过吸附技术对重症急性胰腺炎(SAP)病人具有稳定内环境和保护肝肾功能作用。Objective To study the effect of the coupled plasma filtration adsorption（CPFA）on homeostasis and function of liver and kidney in patients with severe acute pancreatitis（SAP）.Methods Fifty-two patients with severe acute pancreatitis were randomly divided into the observation group and the control group.The control group received normal treatment for ICU,while the observation group received CPFA besides normal treatment.We observed the changes of liver and kidney function,the APACHEⅡscore,and the changes of homeostasis.Results Three days and 7 days after the treatment,leukocyte counting,the plasma liver function index（AST,ALT,GGT,ALB and STB）levels and kidney function index（BUN,and Scr）levels,and APACHE II score were detected in both groups,and the difference was statistically significant（P〈0.05）.Conclusion CPFA therapy can effectively protect the function of liver and kidney in SAP patients,and also improve their overall condition.It was an important auxiliary treatment technique for SAP patients,so it＇s worthy of clinical application.
