摘 要:目的 探讨在单孔胸腔镜术前应用微弹簧圈定位肺内小结节的可靠性和疗效.方法 回顾性分析2016年7月—2017年7月东南大学附属徐州市中心医院进行胸腔镜手术治疗的40例肺结节患者的临床资料.术前在C T引导下,应用微弹簧圈标记拟切除的共47处小结节,再进行单孔胸腔镜下肺楔形切除术,如病理恶性再行肺叶切除并淋巴结清扫术.结果 全部小结节均在术前标记成功,无弹簧圈移位,所有病变均完整切除并获得病理诊断.无中转开胸手术.术后并发症包括肺部感染1例、肺不张1例、肺组织漏气1例.结论 单孔胸腔镜术前,在CT引导下应用微弹簧圈定位周围型肺内小结节是可靠、有效的,有助于迅速、精准切除病灶.Objective To investigate the reliability and efficacy of microcoil for the localization of small pulmonary nodules before uniport thoracoscopic surgery. Methods The clinical information of 40 patients with pulmonary nodules who underwent thoracoscopic surgery betweeu July, 2016 and July, 2017 in Central Hospital, Xuzhou, were retrospectively analyzed. There were 47 peripheral pulmonary nodules in these patients, which were preoperatively localized by CT-guided percutaneous insertion of microcoil. Subsequently, the uniport thoracoscopic wedge resection was carried out. Lobectomy and mediastinal lymph node dissection would be performed if the nodule is tumor-positive. Results All these nodules were localized successfully without translocation under thoracoscopy, and they were resected completely, followed by pathological diagnosis. The postoperative complications include 1 case of pneumonia, 1 case of atelectasis and 1 case of air leakages. Conclusion Before uniport thoracoscopic surgery, CT-guided localization of small pulmonary nodules by microcoil is reliable and efficient for precise resection.
