腹腔镜超声定位与常规腹腔镜方法切除肾脏肿瘤的疗效比较

摘　　要：目的 探索腹腔镜超声在经腹肾脏肿瘤定位肾单位部分切除的使用价值.方法 本院自2013年2月至2016年3月,对8例肾肿瘤患者行腹腔镜超声辅助肾肿瘤切除术,术中使用腹腔镜超声对肿物部位、边界、大小进行实时观测,对切缘进行检测.统计手术持续时间、手术出血量、病理标本切缘情况等指标,并与对照组比较.结果 手术成功,观察组手术持续时间、肾动脉阻断时间、住院时间均优于对照组,手术失血量、术后饮食恢复所需时间、术后肌酐水平较常规手术无明显差异.结论 腹腔镜下超声对肾实质稍小肿瘤具有良好的动态监测作用,一方面可以防止正常肾组织过多切除,另一方面也降低了切缘阳性率.Objective To explore the application of laparoscopic ultrasound in laparoscopic transabdominal ultrasound kidney tumor localization partial nephrectomy.Methods From February 2013 to March 2016,8 patients with renal tumor in our hospital underwent ultrasound assisted laparoscopic nephrectomy.Tumor location,boundaries,size,real-time observation and margin testing were performed by laparoscopic ultrasound in the operation.The operative duration,blood loss,pathological specimens margin status and other indicators were recorded and compared between the two groups.Results All operations were successful.Surgery duration,renal artery occlusion time,hospital stays in the observation group were better than the control group.There was no significant difference in blood loss,postoperative recovery time diet,postoperative creatinine levels between the two groups.Conclusions Laparoscopic ultrasonography in smaller renal tumors has a good dynamic monitoring role,which can prevent the normal kidney tissue ablation excessive and reduce the positive margin rate.
