血清铁蛋白水平与脑梗死发生的相关性研究

摘　　要：目的：探讨血清铁蛋白水平与脑梗死发生的相关性.方法：选取山东大学附属济南市中心医院脑梗死患者336例为研究组,健康体检人群367例为对照组.取2ml空腹促凝血,蛋白芯片法测定血清铁蛋白浓度,利用SPSS 23.0软件统计分析.结果：脑梗死组血清铁蛋白浓度明显高于对照组[（93.8±65.0）ng/ml,（115.7±102.6）ng/ml,P=0.001].二元logistic回归分析发现血清铁蛋白浓度脑与梗死发生成正相关（OR=0.003,P=0.001）,且性别为男性与脑梗死发生成正相关（P〈0.05）.血清铁蛋白与脑梗死诊断进行ROC曲线分析,血清铁蛋白诊断心肌梗死的曲线下面积（AUC）为0.53（渐近95%置信区间：0.417~0.756）;最佳临界点为190.76 mg/ml,结论：脑梗死患者血清铁蛋白水平高于正常对照组,但血清铁蛋白用于脑梗死临床诊断意义较小.Objective： To investigate the correlation between serum ferritin and cerebral infarction. Methods： Three hundred and thirty-six patients with cerebral infarction inJi＇nan Central Hospital Affiliated to Shandong University were selected as the study group, and 367 healthy people were selected as the control group. The anticoagulant blood （2ml, fasting） was collected, then protein chip was used to assay the serum ferritin concentration. SPSS 23.0 software was used for statistical analysis. Results： The serum ferritin concentration in the cerebral infarction group （（115.7 ＋ 102.6） ng/ml） was significantly higher than that in the control group （（93.8 ＋ 65） ng/ml] （P= 0.001） . Bivariate logistic regression analysis showed that serum ferritin concentration was positively related with cerebral infarction （OR.= 0.003, P=0.001） , and the analysis about gender showed that male was positively related to cerebral infarction （P〈0.05） . We prepared the R.OC curve of serum ferritin in the diagnosis of myocardial infarction, and the area under curve （AUC） was 0.53 （95% confidence interval： 0.417 ~ 0.756） ; the best critical point was 190.76 mg/ml. Conclusion： The level of serum ferritin in cerebral infarction patients are higher than that in the control group, but the significance for serum ferritin in cerebral infarction diagnosis was limited.
